介入超声经皮穿刺活检技术在乳腺肿瘤良恶性诊断中的应用策略

摘　　要：目的:分析在临床对乳腺肿瘤良恶性进行诊断中,介入超声经皮穿刺活检技术的实际应用情况。方法:运用回顾性研究方法,对2020年2月~2023年7月诊治的86名乳腺肿瘤患者给予介入超声经皮穿刺活检,并将其作为研究对象,记录超声特征并判断诊断价值。结果:在86例患者中,术中病理判断为恶性肿瘤46例(恶性组),占比53.49%;良性肿瘤40例(良性组),占比46.51%。记录恶性组肿瘤的相关超声形态特征,如最大直径、形态、内部回声、边界等,并与良性组进行对比,其对比差异有统计学意义(P<0.05)。彩色多普勒超声显示,良性组血流分级为Ⅰ型、Ⅱ型、Ⅲ型、Ⅳ型的病例数依次为35例、3例、2例与0例,恶性组血流分级的病例数依次为4例、12例、16例、10例,两组对比有明显差异(P<0.05)。在86例患者中,介入超声经皮穿刺活检诊断为恶性肿瘤45例,良性肿瘤41例,介入超声经皮穿刺活检技术在乳腺肿瘤良恶性诊断中的敏感性、特异性、准确性分别为95.65%(44/46)、97.50%(39/40)、96.51%(83/86)。结论:在乳腺肿瘤良恶性的诊断中,应用介入超声经皮穿刺活检技术能获得良好的敏感性、特异性、准确性。Objective:To explore and analyze the practical application of interventional ultrasound percutaneous biopsy technology in the diagnosis of benign and malignant breast tumors in clinical practice.Methods:A retrospective study was conducted on 86 breast cancer patients who were treated in hospitals from February 2020 to July 2023.All patients were given interventional ultrasound percutaneous puncture biopsy to record ultrasound features and assess diagnostic value.Results:Among 86 patients,46 cases(malignant group)were pathologically diagnosed as malignant tumors during surgery,accounting for 53.49%.There were 40 cases of benign tumors(benign group),accounting for 46.51%.Record the tumor related ultrasound morphological characteristics of the malignant group,such as maximum diameter,shape,internal echo,boundary,etc.,and compare them with the benign group,and the difference in comparison is statistically significant(P<0.05).Color Doppler ultrasound showed that the number of cases with blood flow grading of typeⅠ,typeⅡ,typeⅢ,and typeⅣin the benign group was 35,3,2,and 0,respectively.The number of cases with blood flow grading in the malignant group was 4,12,16,and 10,respectively.There was a significant difference between the two groups(P<0.05).Among 86 patients,45 were diagnosed as malignant tumors and 41 as benign tumors through interventional ultrasound percutaneous biopsy.The sensitivity,specificity,and accuracy of interventional ultrasound percutaneous biopsy technology in the diagnosis of benign and malignant breast tumors were 95.65%(44/46),97.50%(39/40),and 96.51%(83/86),respectively.Conclusion:In the diagnosis of benign and malignant breast tumors,the application of interventional ultrasound percutaneous puncture biopsy technology can achieve good sensitivity,specificity,and accuracy.
