摘  要:目的:探究实时三维超声输卵管造影(RT-3D-HyCoSy)在不孕症患者输卵管病变诊断中的应用价值。方法:选择2020年1月~2022年5月于本院接受诊治的不孕症患者85例为观察对象,均在月经干净后5~7d接受RT-3D-HyCoSy筛查及X射线造影筛查,之后接受腹腔镜手术干预。将腹腔镜手术结果视作诊断金标准,观察并对比两种造影方法的图像质量,并对比分析两种造影方法对输卵管通畅性的筛查结果及诊断效能。结果:RT-3D-HyCoSy筛查的图像质量优良率高于X射线造影(P<0.05);RT-3D-HyCoSy筛查方法对输卵管阻塞的检出率高于X射线造影(P<0.05);85例不孕症患者经腹腔镜手术诊断发现输卵管阻塞者50例、通畅/通而不畅者35例,RT-3D-HyCoSy筛查方法对不孕症患者输卵管通畅性诊断的敏感性、准确性均高于X射线造影(P<0.05)。结论:RT-3D-HyCoSy在不孕症患者输卵管病变诊断中价值较高,可提高诊断精确性,为后期临床防治提供准确依据。Objective:To explore the application value of real-time three-dimensional ultrasound salpingography(RT-3D-HyCoSy)in the diagnosis of tubal lesions in infertility patients.Methods:A total of 85 infertility patients who received treatment in our hospital from January 2020 to May 2022 were selected as the observation subjects.All of them received RT-3D-HyCoSy screening and X-ray screening 5-7 days after menses were cleared,and then received laparoscopic surgical intervention.The results of laparoscopic surgery were regarded as the diagnostic gold standard.The image quality of the two angiography methods was observed and compared,and the screening results and diagnostic efficacy of the two angiography methods for fallopian tube patency were compared and analyzed.Results:The image quality of RT-3D-HyCoSy screening was higher than that of X-ray contrast(P<0.05).The detection rate of RT-3D-HyCoSy screening method for tubal obstruction was higher than that of X-ray imaging(P<0.05).Among the 85 patients with infertility,50 cases were found to have fallopian tube obstruction and 35 cases were found to have patency or obstruction.The sensitivity and accuracy of RT-3D-HyCoSy screening method in diagnosing fallopian tube patency in infertility patients were higher than that of X-ray imaging(P<0.05).Conclusion:RT-3D-HyCoSy has a high value in the diagnosis of tubal lesions in infertility patients,which can improve the diagnostic accuracy and provide accurate basis for the later clinical prevention and treatment.
