摘　　要：目的:探讨动态心电图在监测心房颤动合并H型高血压患者电生理昼夜节律变化方面的临床应用价值。方法:选取2023年1月~2023年9月本院心内科收治的60例心房颤动合并H型高血压患者作为研究对象。所有患者均接受动态心电图监测,观察并记录患者电生理昼夜节律变化情况。结果:患者24h内动态心电图监测到的心房颤动发作次数存在明显昼夜差异,昼间发作次数明显高于夜间;而在心房颤动持续时长方面,22:00~24:00时间段最高,平均发作时间110min,其次是14:00~16:00,平均发作时间97.97min,低谷在06:00~08:00,平均发作时间76.69min;在心电图异常方面,以室性早搏和快速心室率为主。结论:动态心电图监测可有效反映心房颤动合并H型高血压患者电生理昼夜节律变化,为临床诊断和治疗提供重要依据。Objective:To explore the clinical application value of dynamic electrocardiogram in monitoring the electrophysiological circadian rhythm changes in patients with atrial fibrillation and H-type hypertension.Methods:60 patients with atrial fibrillation and H-type hypertension admitted to the cardiology department of our hospital from January 2023 to September 2023 were selected as the study subjects.All patients underwent dynamic electrocardiogram monitoring to observe the changes in their electrophysiological circadian rhythm.Results:There was a significant day night difference in the number of atrial fibrillation episodes detected by the patient's dynamic electrocardiogram within 24 hours,with daytime episodes significantly higher than nighttime episodes;In terms of the duration of atrial fibrillation,the time period from 22:00 to 24:00 was the highest,with an average onset time of 110 minutes,followed by 14:00 to 16:00,with an average onset time of 97.97 minutes.The trough was from 06:00 to 08:00,with an average onset time of 76.69 minutes;In terms of electrocardiogram abnormalities,ventricular premature beats and rapid ventricular rate are the main causes.Conclusion:Dynamic electrocardiogram monitoring can effectively reflect the electrophysiological circadian rhythm changes in patients with atrial fibrillation and H-type hypertension,providing important basis for clinical diagnosis and treatment.
