阴道超声联合腹部超声诊断宫外孕的临床价值及检出率分析

摘　　要：目的:分析在宫外孕诊断中联合应用阴道超声和腹部超声的临床价值。方法:纳入本院2021年1月~2022年12月收治的80例疑似宫外孕患者,均行阴道超声、腹部超声检查,以临床诊疗结果[诊断性刮宫、腹腔镜(开腹)手术病理结果]为金标准,比较两种检查手段单一及联合检查的诊断价值。结果:金标准检出阳性77例、阴性3例;单一经腹部超声检出阳性70例、阴性10例;单一经阴道超声检出阳性71例、阴性9例;联合诊断阳性76例、阴性4例。联合诊断宫外孕类型检出率优于单一腹部超声与单一阴道超声(P<0.05);且单一腹部超声与单一阴道超声对比无差异(P>0.05)。联合诊断灵敏度与准确性显著优于单一腹部超声与单一阴道超声(P<0.05);单一腹部超声与单一阴道超声各指标比较均无差异(P>0.05)。联合诊断图像检出率高于单一腹部超声、单一阴道超声,且单一阴道超声检出率高于单一腹部超声(P<0.05)。联合诊断对孕囊直径≤20mm的检出率优于单一腹部超声、单一阴道超声(P<0.05);单一腹部超声与单一阴道超声比较无差异(P>0.05)。结论:在宫外孕诊断中联合应用阴道超声和腹部超声可提升诊断准确性,为疾病诊断、治疗提供科学参考。Objective:To analyze the clinical value of combining vaginal ultrasound and abdominal ultrasound in the diagnosis of ectopic pregnancy.Methods:80 suspected ectopic pregnancy patients admitted to our hospital from January 2021 to December 2022 were included.All patients underwent vaginal ultrasound and abdominal ultrasound examinations,and the clinical diagnosis and treatment results[diagnostic curettage and laparoscopic(open)surgical pathology results]were used as the gold standard.The diagnostic value of the two examination methods,single and combined,was compared.Results:77 cases were positive and 3 cases were negative according to the gold standard;70 cases were positive and 10 cases were negative detected by single transabdominal ultrasound;71 cases were positive and 9 cases were negative detected by single transvaginal ultrasound;76 cases were positive and 4 cases were negative for joint diagnosis.The detection rate of combined diagnosis of ectopic pregnancy types is better than that of single abdominal ultrasound and single vaginal ultrasound(P<0.05);And there was no difference between single abdominal ultrasound and single vaginal ultrasound(P>0.05).The sensitivity and accuracy of combined diagnosis were significantly better than those of single abdominal ultrasound and single vaginal ultrasound(P<0.05);There was no difference in various indicators between single abdominal ultrasound and single vaginal ultrasound(P>0.05).The detection rate of combined diagnostic images was higher than that of single abdominal ultrasound and single vaginal ultrasound,and the detection rate of single vaginal ultrasound was higher than that of single abdominal ultrasound(P<0.05).The detection rate of combined diagnosis for gestational sac diameter≤20mm is better than that of single abdominal ultrasound and single vaginal ultrasound(P<0.05);There was no difference between single abdominal ultrasound and single vaginal ultrasound(P>0.05).Conclusion:The combination of vaginal ultrasound and abdominal ultrasound in the diagnosi
