摘  要:目的:探讨机器人辅助下后路短节段固定在胸腰椎骨折治疗中的疗效及安全性。方法:回顾性分析2018年6月~2023年5月收治的52例单节段胸腰椎压缩性骨折患者的临床资料,根据手术方式不同分为观察组(机器人辅助经皮椎弓根螺钉内固定术共24例),对照组(骨折切开复位椎弓根螺钉内固定术共28例),比较两组置钉准确率、椎体高度恢复率、术中出血量、手术时间的差异。结果:两组切口愈合良好,均无切口及深部组织感染及医源性神经损伤情况发生。观察组在置钉准确率、术中出血量方面优于对照组,在椎体高度恢复率方面低于对照组,P<0.05,差异具有统计学意义;手术时间比较,P>0.05,无统计学差异。结论:机器人辅助经皮椎弓根螺钉内固定术及骨折切口复位内固定术针对无神经症状的胸腰椎骨折,均可达到恢复稳定的目的,获得满意疗效,但在手术创伤、椎体复位效果及置钉精确度方面存在差异。因此,术者仍需参考手术目的及患者意愿进行手术方式的选择。Objective:To investigate the efficacy and safety of robot-assisted posterior segment fixation in the treatment of thoracolumbar fracture.Methods:Retrospective analysis of 52 clinical data of patients with single level thoracolumbar compression fracture from June 2018 to May 2023,divided into observation group(robot assisted percutaneous pedicle screw fixation 24 cases),control group(28 cases of open reduction pedicle screw fixation),compare the two groups of nailing accuracy,vertebral height recovery rate,intraoperative bleeding,and operation time.Results:The incision healed well without incision and deep tissue infection and iatrogenic nerve injury.The nail accuracy and intraoperative bleeding in observation group were better than control group,and the vertebral height recovery rate was lower than control group,P<0.05,statistically significant;Compared with operation time,P>0.05,no statistical difference.Conclusion:Roboticassisted percutaneous pedicle screw internal fixation and fracture incision reduction internal fixation can achieve stability and achieve satisfactory results,but there are differences in surgical trauma,vertebral reduction effect and nailing accuracy.Therefore,the operator still needs to refer to the purpose of the operation and the patient’s willingness to make the choice of the surgical mode.
