摘　　要：目的:在异位妊娠诊断中,分析异位妊娠超声图像特点,并对阴道联合腹部超声诊断价值进行评估。方法:选取2020年1月~2022年10月高安市骨伤医院超声科66例异位妊娠患者资料,抛币法分为对照组(腹部超声诊断)、研究组(阴道联合腹部超声诊断),各33例。以病理学检查为金标准,对上述诊断措施的实施价值进行评估。结果:异位妊娠超声图像特点:①胎囊型:孕囊周围为强回声环,包块内混杂孕囊样结构回声,有点状血流;②包块型:以低回声为主,表现为混杂回声包块;③盆腔积液型:游离无回声暗区,盆腔内不等量积液。在各类型异位妊娠的疾病检出率分析中,研究组显著高于对照组,差异有意义(P<0.05);研究组阴性预测值、准确度、灵敏度均较高,且与对照组相比存在显著优势,统计学对比有意义(P<0.05)。结论:阴道联合腹部超声诊断价值较高,可以根据异位妊娠超声图像特点,提升疾病检出率,且联合诊断的阴性预测值、准确度、灵敏度高,可以科学指导后续治疗。Objective:In the diagnosis of ectopic pregnancy,the characteristics of ultrasound images of ectopic pregnancy were analyzed,and the diagnostic value of vaginal ultrasonography combined with abdominal ultrasonography was evaluated.Methods:Data of 66 patients with ectopic pregnancy in the ultrasound department of Gaoan Bone Injury Hospital from January 2020 to October 2022 were selected and divided into control group(abdominal ultrasound diagnosis)and study group(vaginal combined abdominal ultrasound diagnosis)with 33 cases each by coin toss method.Taking pathological examination as the gold standard,the value of the above diagnostic measures was evaluated.Results:The characteristics of ectopic pregnancy were as follows:①Fetal sac type:there were strong echo rings around the pregnancy sac,echoes of the pregnancy sac structure mixed in the mass,and punctured blood flow;②Packet type:mainly low echo,manifested as mixed echo packet;③Pelvic effusion type:free echoless dark area,unequal amount of fluid in the pelvic cavity.The detection rate of ectopic pregnancy in the study group was significantly higher than that in the control group(P<0.05).The negative predictive value,accuracy and sensitivity of the study group were higher,and there were significant advantages compared with the control group,the statistical comparison was significant(P<0.05).Conclusion:Vaginal combined abdominal ultrasound has high diagnostic value,which can improve the disease detection rate according to the ultrasonic image characteristics of ectopic pregnancy,and the negative predictive value,accuracy and sensitivity of the combined diagnosis are high,which can scientifically guide the follow-up treatment.
