探讨3.0TMR单侧髋关节扫描与常规扫描对髋关节撞击综合征(FAI)的诊断价值

摘　　要：目的:比较不同扫描方式对髋关节撞击综合征(FAI)的临床分析和价值,并分析单侧髋关节斜位扫描结合360°放射状扫描方法在诊断髋关节撞击综合征中是否具有更高的价值。方法:收集46例患有髋臼盂唇损伤的患者,并对其进行3.0T MR常规扫描和单侧髋关节斜位扫描。以关节镜检查作为诊断标准,比较两种检查方式在确诊髋臼盂唇损伤和FAI方面的效果和价值。结果:常规MR扫描的髋臼盂唇损伤的敏感性、准确率和特异度分别为66.90%、69.00%和82.50%,而单侧髋关节斜位扫描则分别达到了88.60%、90.00%和93.00%,放射状扫描数据则分别达到了94.50%、95.33%和96.30%。相比于常规扫描,单侧髋关节斜位扫描结合360˚放射状扫描能够更加生动清晰地表现出髋臼盂唇的解剖结构,从而更好地诊断和治疗髋臼盂唇损伤,并显著提高了确诊髋臼盂唇损伤的敏感度和阳性率。结论:在3.0T MR扫描中,单侧髋关节斜位扫描结合360°放射状扫描对于诊断髋关节撞击综合征具有更高的价值。Objective:To compare the clinical analysis and value of different scanning methods in the diagnosis of femoroacetabular impingement(FAI),and analyze whether unilateral oblique hip scan combined with 360° radial scan has higher value in the diagnosis of hip impingement syndrome.Methods:46 patients with acetabular labial injury were examined by 3.0T MR scan and unilateral hip oblique scan.Arthroscopy was used as the diagnostic standard to compare the efficacy and value of the two methods in the diagnosis of acetabular labial injury and FAI.Results:The sensitivity,accuracy and specificity of conventional MR Scan were 66.90%,69.00%and 82.50%,while those of unilateral oblique scan were 88.60%,90.00%and 93.00%,and those of radial scan were 94.50%,95.33%and 96.30%,respectively.Compared with conventional scan,unilateral hip oblique scan combined with 360˚radial scan can more vividly show the anatomical structure of the acetabular labrum,so as to better diagnose and treat the injury of the acetabular labrum,and significantly improve the sensitivity and positive rate of diagnosis of the injury of the acetabular labrum.Conclusion:In 3.0T MR scan,unilateral oblique hip scan combined with 360˚radial scan has higher value in the diagnosis of hip impingement syndrome.
