口腔颌面部基于CBCT及Simplant软件观察下颌管分支情况的研究

摘　　要：目的:探讨口腔颌面部基于CBCT及Simplant软件观察下颌管分支情况。方法:选取2021年2月~2023年6月本院口腔科行CBCT检查的患者90例(180侧),统计其下颌管分支情况及类型,对CBCT及Simplant软件的诊断效能进行分析。结果:90例(180侧)患者中,共有40侧(22.22%)存在分支情况,其中Ⅰ型、Ⅱ型、Ⅲ型、Ⅳ型分别有18侧、13侧、0侧、9侧;而在男性中有25侧(13.89%)存在分支情况,女性患者中15侧(8.33%)存在分支情况。在本组的90例患者中,有37例患者(41.11%)有下颌管分支,Ⅰ型、Ⅱ型、Ⅲ型、Ⅳ型分别有16例、12例、0例、9例。其中诊断Ⅰ型敏感度为93.75%、特异度为95.24%、准确度为94.59%、阳性预测值为93.75%、阴性预测值为95.24%;诊断Ⅱ型敏感度为91.67%、特异度为96.00%、准确度为94.59%、阳性预测值为91.67%、阴性预测值为96.00%;诊断Ⅳ型敏感度为88.89%、特异度为100.00%、准确度为97.30%、阳性预测值为100.00%、阴性预测值为96.56%。结论:通过CBCT及Simplant软件可提高下颌管分支检出率。Objective:To investigate the branches of jaw branch based on CBCT and Simplant software.Methods:Selected 90 patients(180 side)with CBCT in the Department of Stomatology from February 2021 to June 2023,collected the branch status and type of mandibular duct,and analyzed the diagnostic efficacy of CBCT and Simplant software.Results:Among 90 patients(180 sides),a total of 40 sides(22.22%)had branching,with 18 sides,13 sides,0 sides,and 9 sides of Type Ⅰ,Ⅱ,Ⅲ,and Ⅳ types,respectively;Among males,25 sides(13.89%)had branching,while among females,15 sides(8.33%)had branching.Among the 90 patients in this group,37 patients(41.11%)had mandibular canal branching,and there were 16 cases,12 cases,0 cases,and 9 cases of type Ⅰ,Ⅱ,Ⅲ,and Ⅳ,respectively.The sensitivity of type Ⅰ diagnosis is 93.75%,specificity is 95.24%,accuracy is 94.59%,positive predictive value is 93.75%,and negative predictive value is 95.24%;The sensitivity of type Ⅱ diagnosis is 91.67%,specificity is 96.00%,accuracy is 94.59%,positive predictive value is 91.67%,and negative predictive value is 96.00%;The sensitivity of type Ⅳ diagnosis is 88.89%,specificity is 100.00%,accuracy is 97.30%,positive predictive value is 100.00%,and negative predictive value is 96.56%.Conclusion:The detection rate of mandibular tube branch can be improved by CBCT and Simplant software.
