摘 要:目的运用孟德尔随机化模型探索肌酐水平对椎间盘退行性疾病的影响。方法利用双向孟德尔随机化方法,评估肌酐水平与椎间盘退行性疾病之间的相关性。采用两个独立的GWAS公共遗传数据库,在孟德尔随机化分析中,以逆方差加权法的结果作为主要评价指标,另外4种方法作为参考;Cochrans’Q异质性检测、孟德尔随机化Egger-intercept检测和孟德尔随机化-PRESSO法用于检测异质性和多效性;留一法、敏感性森林图、漏斗图用于检测敏感性;Radial-孟德尔随机化法用于检测异常值。结果肌酐水平对椎间盘退行性疾病存在显著的正向因果效应[逆方差加权法:OR=1.218;95%CI:(1.085,1.368);P=0.001,<0.05]。椎间盘退行性疾病对肌酐不存在因果效应[逆方差加权法:OR=0.961;95%CI:(0.923,1.001);P=0.055,>0.05],异质性检测表明无显著偏倚存在(P=0.081,>0.05),水平多效性检测显示低多效性偏倚(P=0.471,>0.05)。结论肌酐水平升高是椎间盘退行性疾病的潜在不利因素。Objective Utilizing the Mendelian randomization model to explore the impact of creatinine levels on Degenerative intervertebral disc disease.Methods Using bidirectional Mendelian randomization,this section evaluates the correlation between creatinine levels and Degenerative intervertebral disc disease.This study employed two independent public genetic databases from Genome-Wide Association Studies(GWAS),this study primarily evaluates the causal relationship using the results of the Inverse-variance weighted method in Mendelian randomization analysis,with four additional methods serving as references.Cochrans'Q test for heterogeneity,Mendelian randomization Egger-intercept test,and Mendelian randomization-PRESSO method employed for detecting heterogeneity and pleiotropy;Leave-one-out analysis and sensitivity,with forest plots and funnel plots utilized for sensitivity detection;Radial-Mendelian randomization approach used for outlier detection.Results Creatinine levels showed a significant positive causal effect on Degenerative intervertebral disc disease[IVW:OR=1.218;95%CI:(1.085,1.368);P=0.001,<0.05].Degenerative intervertebral disc disease has no causal effect on creatinine levels[IVW:OR=0.961;95%CI:(0.923,1.001);P=0.055,>0.05].Heterogeneity tests indicated no significant bias(P=0.081>0.05),and Pleiotropy tests showed a low level of bias(P=0.471,>0.05).Conclusion Elevated creatinine levels are a potential risk factor for degenerative intervertebral disc disease.
