基于肠道菌群探讨从瘀论治IgA肾病

摘　　要：IgA肾病是我国最常见的一种原发性肾小球肾炎疾病,是导致终末期肾脏病的主要原因之一。血瘀为该病进展的关键病机,因此,活血化瘀应贯穿于治疗全程。血瘀证涵盖现代多种血流动力学的改变,肠道菌群失调可通过影响血流动力学、炎症反应等途径导致血液循环障碍、血小板聚集、血栓前状态促进IgA肾病的发生发展。应用活血化瘀药物可改善肠道菌群的紊乱,延缓IgA肾病向终末期肾脏病的进展。IgA nephropathy(IgAN)is the most common primary glomerulonephritis disease in China,which is one of the main causes of end-stage renal disease.Blood stasis is the key pathogenesis of the disease progression.Therefore,activating blood circulation and resolving stasis should be carried out throughout the treatment process.Blood stasis syndrome covers a variety of modern hemodynamic changes.Imbalance of gut microbiota can lead to blood circulation disorders,platelet aggregation,prethrombotic state by affecting hemodynamics,inflammatory response and other ways,thus inducing the occurrence and development of IgAN.The application of traditional Chinese medicine for activating blood circulation and resolving stasis can improve the disorder of gut microbiota and delay the progression of IgAN to end-stage renal disease.
