逐痰祛瘀汤联合奥美沙坦酯治疗痰瘀互结型高血压

摘　　要：目的:观察逐痰祛瘀汤联合奥美沙坦酯治疗痰瘀互结型高血压的临床疗效及其对可溶性细胞间黏附分子-1(soluble intercellular adhesion molecule-1,sICAM-1)、同型半胱氨酸(homocysteine,Hcy)水平的影响。方法:80例痰瘀互结型高血压患者随机分为对照组(40例)和观察组(40例)。对照组给予奥美沙坦酯,观察组在对照组治疗基础上给予逐痰祛瘀汤,两组患者均治疗8周。比较两组患者的临床疗效、中医证候积分、舒张压、收缩压、sICAM-1、Hcy、C-反应蛋白(C-reactive protein,CRP)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞介素-6(interleukin-6,IL-6)及不良反应。结果:观察组有效率为97.50%,高于对照组的85.00%(P<0.05)。治疗后,两组患者的中医证候积分、舒张压及收缩压均显著降低,且观察组中医证候积分、舒张压及收缩压均显著低于同期对照组(P<0.05)。治疗后,两组患者sICAM-1、Hcy水平均显著降低,且观察组患者sICAM-1、Hcy水平均低于同期对照组(P<0.05)。治疗后,两组患者血清CRP、TNF-α及IL-6水平均显著降低,且观察组患者血清CRP、TNF-α及IL-6水平均显著低于同期对照组(P<0.05)。两组患者头晕、食欲不振及呕吐等不良反应发生率比较,差异无统计学意义(P>0.05)。结论:逐痰祛瘀汤联合奥美沙坦酯治疗痰瘀互结型高血压疗效确切,能够保护血管内皮细胞功能和抑制炎症反应,且安全性较好。Objective:To observe the clinical efficacy of Zhutan Quyu Decoction combined with Olmesartan Medoxomil Tablets in the treatment of hypertension with Phlegm-stasis congestion syndrome as well as its effect on the expression levels of soluble intercellular adhesion molecule-1(sICAM-1)and homocysteine(Hcy).Methods:A total of 80 patients with hypertension of Phlegm-stasis congestion syndrome were randomly divided into a control group(40 cases)and an observation group(40 cases).The control group was given Olmesartan Medoxomil Tablets,while the observation group was given Zhutan Quyu Decoction based on the treatment of the control group.Both groups were treated for 8 weeks.The clinical efficacy,TCM syndrome score,diastolic blood pressure,systolic blood pressure,sICAM-1,Hcy,C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and adverse reactions were compared between the two groups.Results:The effective rate was 97.50%of the observation group,which was higher than 85.00%of the control group significantly(P<0.05).After treatment,the TCM syndrome score,diastolic blood pressure and systolic blood pressure of the two groups were significantly reduced,and those of the observation group were significantly lower than those of the control group during the same period(P<0.05).In addition,the sICAM-1 and Hcy levels of the two groups were significantly decreased,and the levels of the observation group were significantly lower than those of the control group(P<0.05).Besides,the levels of serum CRP,TNF-αand IL-6 of the two groups were significantly decreased,and the levels of the observation group were significantly lower than those of the control group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions such as dizziness,loss of appetite and vomiting between the two groups(P>0.05).Conclusion:Zhutan Quyu Decoction combined with Olmesartan Medoxomil Tablets is effective in the treatment of hypertension with Phlegm-stasis congestion syndrome,which can protect the fu
