甘温除热法治疗乳腺癌化疗后并发症

摘　　要：乳腺癌的发病以气郁、痰凝、血瘀为因,患之日久,积聚为核,囿于乳络,终耗气伤血,应用化疗药物后,毒邪进入体内最先伤及脾胃,脾胃受损则气血生化无本,输布无源,进而影响其他脏腑生理功能,致正气虚损、气血耗伤。甘温除热法可扶正祛邪、调畅气机、补益气血、安和诸脏,病及于心可用泻火升阳汤,病及于肝可用补中益气汤,病及于肺可用升阳益胃汤,病及于肾可用神圣复气汤。临证时需把握甘温除热法对应证的病机变化,选用方除李东垣著作中所载方外,亦可选黄芪建中汤、人参养荣汤、十全大补汤、七味白术散等符合甘温除热思想之方。The onset of breast cancer is due to qi stagnation,phlegm coagulation,and blood stasis.The accumulation of these pathogenic factors is the core of the disease.Over time,the pathogens accumulate into a nucleus which is confined to the mammary meridians,and eventually consume qi and damage blood.After the application of chemotherapy drugs,the toxins enter the body and first hurt the spleen and stomach.If the spleen and stomach are damaged,the qi and blood generation will have no resources,and the distribution will be passive,thus affecting the physiological function of other viscera,causing the deficiency of heathy qi and the consumption of qi and blood.The method of relieving fever with sweet-warm drugs can strengthen the healthy qi and eliminate pathogens,regulate qi movement and nourish qi and blood,and harmonize various organs.When disease involves the heart,Decoction for Purging Yin Fire and Raising Yang can be used.When disease involves the liver,Center-Supplementing and Qi-Boosting Decoction can be used.When disease involves the lung,Yang-Raising and Stomach-Boosting Decoction.When disease involves the kidney,Sacred Qi-Restoring Decoction can be used.In clinical practice,it is necessary to grasp the changes in the pathogenesis of the corresponding syndrome of the method of relieving fever with sweet-warm drugs.In addition to the formulas listed in Li Dongyuan′s works,the selection of formulas can also include Astragalus and Center-Fortifying Decoction,Ginseng Supporting and Nourishing Decoction,Perfect Major Supplementation Decoction,Six-Ingredient Rhizoma Atractylodis Macrocephalae Powder,etc.Any formula that conforms to the method of relieving fever with sweet-warm drugs can be flexibly selected.
