摘　　要：目的分析助产士主导的产前群组化健康管理保健模式在妊娠期甲减患者中的应用效果。方法选择2020年2月至2022年3月收治的184例妊娠期甲减患者为研究对象,以随机数字表法将其分为对照组、观察组,每组92例。两组均给予左甲状腺素替代治疗,对照组在替代治疗基础上给予常规护理,观察组在对照组基础上给予助产士主导的产前群组化健康管理保健模式。比较两组的干预效果。结果干预后,观察组的结果期望、自我效能期望评分高于对照组(P<0.05)。干预后,观察组的情绪管理、日常生活管理、疾病症状管理及疾病认知管理评分均高于对照组(P<0.05)。干预后,观察组的抑郁自评量表(SDS)、焦虑自评量表(SAS)评分低于对照组(P<0.05)。观察组的自然分娩率高于对照组,不良母婴结局总发生率低于对照组(P<0.05)。结论助产士主导的产前群组化健康管理保健模式可应用于妊娠期甲减患者护理中,能够转变其心理状态,提高患者分娩信心及健康管理能力,改善分娩结局。Objective To analyze the application effect of midwives-led prenatal group health management and health care model in patients with hypothyroidism during pregnancy.Methods A total of 184 patients with hypothyroidism during pregnancy admitted from February 2020 to March 2022 were selected as the research objects,and the patients were divided into control group and observation group by random number table method,with 92 cases in each group.Both groups were given levothyroxine replacement therapy,the control group was given routine nursing on the basis of replacement therapy,and the observation group was given midwives-led prenatal group health management and health care model on the basis of the control group.The intervention effects of the two groups were compared.Results After intervention,the scores of outcome expectation and self-efficacy expectation in the observation group were higher than those in the control group(P<0.05).After intervention,the scores of emotional management,daily life management,disease symptom management and disease cognitive management in the observation group were higher than those in the control group(P<0.05).After intervention,the scores of Self-Rating Depression Scale(SDS)and Self-Rating Anxiety Scale(SAS)in the observation group were lower than those in the control group(P<0.05).The natural delivery rate of the observation group was higher than that of the control group,and the total incidence of adverse maternal and infant outcomes was lower than that of the control group(P<0.05).Conclusion The midwives-led prenatal group health management and health care model can be used in the nursing of patients with hypothyroidism during pregnancy,which can change the psychological state,enhance the delivery confidence and health management ability,and improve the delivery outcome.
