基于能力、机会、动机-行为模型的护理干预在血糖控制不良2型糖尿病患者中的应用

摘　　要：目的 探讨基于能力、机会、动机-行为模型(capability, opportunity, motivation-behavior, COM-B)的护理干预在血糖控制不良2型糖尿病患者中的应用效果。方法 选取2021年12月-2022年9月笔者所在医院收治的血糖控制不良的2型糖尿病患者为研究对象,采用随机数字表法将其分为对照组和观察组,各55例。对照组接受常规护理,观察组在对照组基础上给予基于COM-B模型的护理干预,比较2组血糖控制情况、自我效能以及生活质量。结果 干预3个月后,观察组随机血糖和糖化血红蛋白均低于对照组(t=3.380,P<0.001;t=26.907,P<0.001),糖尿病自我效能量表得分高于对照组(t=-12.019,P<0.001),糖尿病生存质量特异性量表得分低于对照组(t=9.899,P<0.001)。结论 基于能力、机会、动机-行为模型的干预能够改善血糖控制不良2型糖尿病患者的血糖水平,提高其自我效能,促进其生活质量的提升。Objective To explore the effect of nursing intervention based on capability opportunity motivation-behavior(COM-B)model on type 2 diabetes patients with poor blood glucose control.Methods Patients found of poor blood glucose control through the physical examination in the author′s hospital between December 2021 and September 2022 were selected.They were divided into a control group and an observation group,each of 55,according to a random number table.Both groups received routine nursing,while the observation group was additionally provided with nursing intervention based on the COM-B model.After the intervention,the blood glucose control,self-efficacy and life quality were compared between the two groups.Results After 3-month intervention,the random blood glucose and glycosylated hemoglobin of the observation group were significantly lower than that of the control group(t=3.380,P<0.001;t=26.907,P<0.001).Moreover,the score of DM self-efficacy scale of the former group was significantly higher than the latter(t=-12.019,P<0.001),and that of diabetes specific scale of quality life was significantly lower than the control group(t=9.899,P<0.001).Conclusion Nursing intervention based on COM-B can control blood glucose level,improve self-efficacy and life quality of type 2 diabetes patients.
