腓动脉穿支蒂接力皮瓣旋转移位修复足踝部及供瓣区皮肤缺损

摘　　要：目的探讨腓动脉穿支“螺旋浆”接力皮瓣旋转移位修复足踝部及供瓣区皮肤软组织缺损的治疗效果。方法2017年6月-2022年10月收治足踝部皮肤软组织缺损患者17例,采用腓动脉穿支“螺旋浆”接力皮瓣修复创面,皮瓣第二供区一期直接缝合15例,2例残留少许创面植皮修复。足踝部软组织缺损范围:6 cm×4 cm~12 cm×8 cm,修复创面的第一皮瓣及供区的第二皮瓣面积分别为:7 cm×5 cm~15 cm×9 cm、6 cm×4 cm~16 cm×6 cm。通过观察受区及供区的功能及外形,评价临床效果。结果所有皮瓣均一期顺利成活,随访3~36个月,平均18个月,皮瓣外观平整、质地良好、色泽接近正常;第二皮瓣供区仅残留线状瘢痕,植皮区皮片成活良好,不影响肢体整体外观。结论腓动脉穿支“螺旋浆”接力皮瓣血供可靠,不损伤主干血管,手术部位均位于小腿后外侧,可有效修复足踝部皮肤软组织缺损及第一皮瓣供区,并获得较好的外观。Objective To investigate the therapeutic effect of peroneal artery perforator propeller relay flap in repairing skin and soft tissue defects in the ankle.Methods From June 2017 to October 2022,17 patients with skin and soft tissue defects of the ankle were treated.Peroneal artery perforator propeller relay flaps were used to repair wounds,and the donor site of the second flap was sutured directly in 15 cases,and 2 patients had a small residual wound and were repaired by skin grafting.The wound area of the soft tissue defect in the ankle ranged from 6 cm×4 cm to 12 cm×8 cm.The areas of the first flap for repairing the wound and the second flap for repairing the donor area of the first flap were 7 cm×5 cm~15 cm×9 cm,6 cm×4 cm~16 cm×6 cm,respectively.Evaluated the clinical effect by observing the function and appearance of the recipient area and the donor area.Results All flaps survived successfully at the first stage.All patients were followed up for 3 to 36 months,with an average of 18 months.The appearance of the flap was smooth,the texture was good,and the color was close to normal;Only linear scars remained in the donor site of the second flap,and the skin graft in the grafted area survived well without affecting the overall appearance of the limb.Conclusion The peroneal artery perforator propeller relay flap has a reliable blood supply and does not damage the main blood vessels.The surgical site is located on the posterolateral side of the calf.It can effectively repair the skin and soft tissue defects of the ankle and the first flap donor area,and obtain a better appearance.
