微型锁定钢板联合空心螺钉内固定对斜形内踝骨折康复效果的影响

摘  要:目的探讨微型锁定钢板联合空心螺钉内固定对斜形内踝骨折患者康复效果的影响。方法选取2020年1月-2022年4月收治的斜形内踝骨折患者94例,依据随机数字表法分为联合固定组及空心螺钉组,各47例。空心螺钉组采取空心螺钉进行内固定,联合固定组采取微型锁定钢板联合空心螺钉进行内固定。比较两组临床疗效、围术期指标(住院时间、术中失血量、手术时长、骨折愈合时间)、踝关节功能(Mazur功能评分及Baired-Jackson踝关节评分)、并发症。结果联合固定组治疗优良率(93.62%)高于空心螺钉组(78.72%)(P<0.05);联合固定组手术时长、术中失血量、住院时间与空心螺钉组比较,差异无统计学意义(P>0.05);联合固定组骨折愈合时间短于空心螺钉组(P<0.05);术后1个月、3个月,联合固定组Mazur及Baired-Jackson评分高于空心螺钉组(P<0.05);联合固定组并发症发生率(4.26%)低于空心螺钉组(17.02%)(P<0.05)。结论微型锁定钢板联合空心螺钉内固定治疗斜形内踝骨折,骨折愈合更快,术后踝关节功能康复效果更好,治疗优良率更高,且并发症发生率更低,具有更好的安全性。Objective To investigate the effect of micro-locking plate combined with hollow screw fixation on the rehabilitation of patients with oblique medial malleolar fracture.Methods A total of 94 patients with oblique medial malleolar fracture admitted to our hospital from January 2020 to April 2022 were selected and divided into micro-locking plate group and hollow screw group according to random number table method,47 cases in each group.The hollow screw group was fixed with hollow screws,and the micro-locking plate group was fixed with micro-locking plate combined with hollow screws.The clinical efficacy,perioperative indexes(hospitalization days,intraoperative blood loss,operation time,fracture healing time),ankle function(Mazur function score and Baired-Jackson ankle score)and complications were compared between two groups.Results The excellence rate of micro-locking plate group(93.62%)was higher than that of hollow screw group(78.72%)(P<0.05).There were no significant differences in operation time,intraoperative blood loss and hospitalization days between microlocking plate group and hollow screw group(P>0.05).The fracture healing time of micro-locking plate group was shorter than that of hollow screw group(P<0.05).The Mazur and Baired-Jackson scores of micro-locking plate group were higher than those of hollow screw group after 1 and 3 months of operation(P<0.05).The complication rate of micro-locking plate group(4.26%)was lower than that of hollow screw group(17.02%)(P<0.05).Conclusion Micro-locking plate combined with hollow screw internal fixation in the treatment of oblique medial malleolar fracture has faster fracture healing,better ankle functional rehabilitation,higher rate of good and good treatment,lower complication rate,and better safety.
