富血小板血浆联合动力化治疗股骨骨折髓内钉固定后延迟愈合

摘　　要：目的探讨以富血小板血浆联合动力化治疗股骨骨折髓内固定后延迟愈合的效果。方法2018年1月-2022年1月将40例股骨骨折髓内固定术后延迟愈合病例分为两组,每组20例。观察组采取富血小板血浆联合动力化治疗,对照组单纯予以动力化治疗。观察术后骨折愈合情况。结果术后1、3、6个月观察组骨痂评分均高于对照组(P<0.05)。观察组骨折全部愈合,平均愈合时间(8.38±2.26)个月;对照组16例愈合,4例未愈合,平均愈合时间(10.27±2.32)个月。结论富血小板血浆联合动力化处理可明显提升股骨骨折髓内固定后延迟愈合愈合率,促进骨痂生长,缩短骨折愈合时间,手术微创,并发症少,治疗效果确切,是一种适合的治疗方案。Objective This study aims to investigate the effects of treating delayed union following intramedullary nailing fixation for femoral fractures with platelet-rich plasma in conjunction with dynamization.Methods From January 2018 to January 2022,40 cases of delayed union after femoral fracture intramedullary fixation surgery were divided into two groups of 20 each.The treatment group received a combination of platelet-rich plasma and dynamization,while the control group was treated with dynamization alone.The postoperative healing of fractures was observed.Results At 1,3,and 6 months postoperatively,the callus scores in the treatment group were significantly higher than those in the control group(P<0.05).All fractures in the treatment group healed,with an average healing time of(8.38±2.26)months;in the control group,4 cases did not heal,and 16 cases healed,with an average healing time of(10.27±2.32)months.Conclusion The combination of platelet-rich plasma and dynamization treatment significantly improves the healing rate of delayed union after intramedullary fixation of femoral fractures,promotes callus growth,and shortens the time to fracture healing.This minimally invasive approach with few complications is an effective treatment option.
