摘 要:目的研究不同电极位点刺激下人工耳蜗植入(CI)儿童的皮层听觉诱发电位(cortical auditory evoked potential,CAEP)与听觉行为分级-Ⅱ(CAP-Ⅱ)、言语可懂度分级标准(SIR)之间的相关性及其影响因素,为CAEP进入临床应用提供依据。方法对26名CI儿童进行不同刺激位点(底转、中转、顶转)下CAEP测试,收集CAP-Ⅱ、SIR得分,对CAEP的P1、N1波与听觉言语量表之间进行相关分析,探究可能的影响因素。结果(1)顶转电极刺激下,植入年龄与P1潜伏期呈正相关(r=0.423,P<0.05);(2)植入年龄与CAP-Ⅱ得分呈负相关(r=-0.520,P<0.01);植入年龄、开机后时间与SIR得分呈负相关(r=-0.409,P<0.05;r=0.481,P<0.05);(3)底转电极刺激下,P1潜伏期与CAP-Ⅱ呈负相关(r=-0.445,P<0.05)。结论CAEP作为能够反映听觉皮层功能的客观评估方式,也能够用于听觉言语能力评估。植入年龄越早、开机后时间越长,听觉言语发育越成熟。Objective To investigate the correlation between cortical auditory evoked potentials(CAEP)and auditory behavior grading-Ⅱ(CAP-Ⅱ),as well as the Speech Intelligibility Rating(SIR)standards in children with cochlear implantation(CI)under different electrode stimulation sites,and their influencing factors,to provide further basis for the clinical application of CAEP.Methods CAEP tests were conducted on 26 CI children under different stimulation sites,and CAP-Ⅱ and SIR scores were collected.A correlation analysis was performed between the P1 and N1 waves of CAEP and the auditory speech scale,and possible influencing factors were explored.Results 1)Under stimulation at the apical electrode,the age of implantation was positively correlated with the P1 latency(r=0.423,P<0.05),2)The age of implantation was negatively correlated with CAP-Ⅱ scores(r=-0.520,P<0.01),the ages of implantation and post-switch-on time were negatively correlated with SIR scores(r=-0.409,P<0.05;r=0.481,P<0.05),3)Under stimulation at the basal electrode,the P1 latency was negatively correlated with CAP-Ⅱ(r=-0.445,P<0.05).Conclusion There is a correlation between CAEP and the auditory speech scale,and different stimulation sites can influence this correlation.The earlier the age of implantation and the longer the post-switch-on time,the more mature the auditory speech development.
