摘　　要：报告1例两种真菌混合感染引起的幼儿头癣。患儿女,3岁。脱发斑伴瘙痒1余年,痒痛加重2个月。皮肤科检查:脱发斑周围头发松动,部分有小脓疱。取松动病发行真菌检查镜下可见发内孢子;脓疱处病发镜下可见发外少量菌丝,发内外大量孢子。真菌培养结果示两种不同形态的菌落生长,核糖体r DNA的内转录间隔区(ITS)分子测序结果分别鉴定为紫色毛癣菌与犬小孢子菌。诊断:幼儿黑点癣;脓癣。治疗:予外用2%咪康唑乳膏及特比萘芬凝胶,连续规律治疗3个月后患儿症状明显改善。随访2年,皮损痊愈未复发。A case of tinea capitis in a child caused by co-infection of two fungi is reported.A 3-year-old girl presented with itchy scalp and hair loss for more than one year and worsened pain for 2 months.Physical examination showed the area of hair loss.The hairs around the area of hair loss were loose and easy to pull out.A few pustules were noted.Microscopic examination of the diseased hair revealed fungal spores within the hair shafts.Microscopic examination of the hairs from pustules showed fungal hyphae outside of the hair shafts as well as fungal spores within and outside of the hair shafts.Trichophyton violaceum and Microsporum canis were identified based on morphology and sequencing with ITS domain,respectively.A diagnosis of black dot and kerion type of tinea capitis in a child caused by co-infection of Trichophyton violaceum and Microsporum canis was made.Topical treatment was carried out by using 2%miconazole nitrate and terbinafine hydrochloride for three months,and scalp lesions improved.The patient healed without relapse over 2 years of follow-up.
