摘  要:目的分析血清Toll样受体4(TLR4)、白细胞介素-25(IL-25)、白细胞介素-33(IL-33)与小儿肺炎支原体肺炎(MPP)病情的相关性及预测价值。方法回顾性选取2022年6月—2023年6月苏州大学附属第二医院收治的150例MPP患儿作为研究对象。根据MPP患儿病情,分为难治性肺炎支原体肺炎(RMPP)组、轻症肺炎支原体肺炎(MMPP)组;根据患儿预后情况,分为预后不良组、预后良好组。患儿入院时均接受血清TLR4、IL-25、IL-33检测,统计患儿基线资料。采用二元logistic回归分析血清TLR4、IL-25、IL-33水平与MPP患儿病情严重程度及预后的关系;绘制受试者工作特征(ROC)曲线,分析血清TLR4、IL-25、IL-33水平对MPP患儿病情严重程度的预测价值。结果RMPP组、预后不良组病变范围≥2个肺叶、病变类型为大片状阴影患儿占比分别高于MMPP组、预后良好组,TLR4相对表达量、IL-25、IL-33水平分别高于MMPP组、预后良好组(P<0.05);二元logistic回归分析结果显示,病变类型为大片状阴影,血清TLR4相对表达量、IL-25、IL-33高表达是MPP患儿病情为MMPP、预后不良的危险因素(OR>1,P<0.05);ROC曲线显示,血清TLR4相对表达量、IL-25、IL-33水平及联合检测预测MPP患儿病情为RMPP的曲线下面积(AUC)均>0.70,有一定的预测价值,其中联合检测最高。结论血清TLR4、IL-25、IL-33水平与MPP患儿病情严重程度及预后密切相关,三者水平能够有效预测MPP患儿病情发展。Objective To analyze the correlation and predictive value of serum Toll-like receptor 4(TLR4),interleukin-25(IL-25)and interleukin-33(IL-33)in children with Mycoplasma pneumoniae pneumonia(MPP).Methods A retrospective study was conducted on 150 cases of children with MPP admitted to the Second Affiliated Hospital of Soochow University from June 2022 to June 2023.Based on the severity of MPP,the children were divided into refractory Mycoplasma pneumoniae pneumonia(RMPP)group and mild Mycoplasma pneumoniae pneumonia(MMPP)group.According to the prognosis of children,they were divided into poor prognosis group and good prognosis group.Serum TLR4,IL-25 and IL-33 were detected in all children at admission,and the baseline data were statistically analyzed.Binary logistic regression was used to analyze the relationship between serum TLR4,IL-25,IL-33 and the severity and prognosis of children with MPP.The receiver operating characteristic(ROC)curve was drawn to analyze the predictive value of serum TLR4,IL-25 and IL-33 levels on the severity of MPP in children.Results The proportion of children with lesion range≥2 lobes and large patchy shadow in RMPP group and poor prognosis group was higher than that in MMPP group and good prognosis group.The relative expression level of TLR4,IL-25 and IL-33 in RMPP group and poor prognosis group was higher than those in MMPP group and good prognosis group(P<0.05).Binary logistic regression analysis showed that the lesion type as large patchy shadows and high expression of serum TLR4,IL-25,and IL-33 were risk factors for MPP children having MMPP and poor prognosis(OR>1,P<0.05).ROC curves showed that the AUC of relative expression of serum TLR4,the levels of IL-25,IL-33,and their combined detection for predicting RMPP in children with MPP were all>0.70,indicating a certain predictive value,which combined detection was the highest.Conclusions The levels of serum TLR4,IL-25 and IL-33 are closely related to the severity and prognosis of children with MPP,and their levels can effectively p
