白芍总苷胶囊联合丁酸氢化可的松乳膏治疗慢性湿疹的效果及对炎症因子的影响

摘　　要：目的:探究白芍总苷胶囊联合丁酸氢化可的松乳膏治疗对慢性湿疹患者皮损面积和炎症反应的影响。方法:选取2021年7月-2022年6月于笔者医院接受治疗的80例慢性湿疹患者,采用单双球法随机分为对照组(丁酸氢化可的松乳膏)和联合组(白芍总苷胶囊联合丁酸氢化可的松乳膏治疗),各40例。比较治疗前和治疗4周后两组患者临床效果[湿疹面积及严重程度指数(Eczema area and severity index,EASI)、中医证候积分、整体美学改善量表(Global aesthetic improvement scale,GAIS)]、皮肤屏障功能[经皮水分丢失值(Transepidermal water loss,TEWL)、湿度、角质层含水量(Stratum corneum hydration,SCH)]、免疫因子(Th1、Th2、IgE)、炎症因子水平(IL-23、IL-17、IL-1β)。结果:治疗4周后,两组患者EASI评分、中医证候积分、TEWL、Th1、Th2、IgE、IL-23、IL-17及IL-1β水平均较治疗前降低,联合组低于对照组,但GAIS高于对照组(均P<0.05);湿度、SCH较治疗前升高,联合组高于对照组(均P<0.05)。结论:白芍总苷胶囊联合丁酸氢化可的松乳膏能够改善慢性湿疹患者临床症状,提高患者整体美观度,增强患者皮肤屏障功能,改善患者免疫功能,减轻患者炎症反应。Objective To explore the influence of total glucosides of paeony capsules combined with hydrocortisone butyrate ointment on skin lesion area and infl ammatory response in patients with chronic eczema.Methods Eighty patients with chronic eczema who received treatment in the hospital from July 2021 to June 2022 were selected and randomly divided into the control group(hydrocortisone butyrate ointment)and the combined group(total glucosides of paeony capsules combined with hydrocortisone butyrate ointment)by single-double ball method,with 40 cases in each group.Clinical effect[Eczema Area and Severity Index(EASI),TCM syndrome score,Global aesthetic improvement scale(GAIS)],skin barrier function[transepidermal water loss(TEWL),humidity,stratum corneum hydration(SCH)],immune function(Th1,Th2,IgE)and infl ammatory factors(IL-23,IL-17,IL-1β)were compared between the two groups before treatment and after 4 weeks of treatment.Results After 4 weeks of treatment,the EASI scores,TCM syndrome score,TEWL,Th1,Th2,IgE,IL-23,IL-17 and IL-1βin both groups were decreased compared with those before treatment,and the above indicators in combined group were lower than those in control group,but the GAIS score was higher than that in control group(all P<0.05).Humidity and SCH were enhanced compared with those before treatment,and the two indicators were higher in combined group than those in control group(all P<0.05).Conclusion Total glucosides of paeony capsules combined with hydrocortisone butyrate ointment can improve the clinical symptoms,enhance the overall aesthetics,promote the skin barrier function,improve the immune function and relieve the infl ammatory response in patients with chronic eczema.
