摘 要:目的分析隐匿性脊髓栓系综合征(occult tethered cord syndrome,OTCS)脑脊液代谢特征、代谢途径和作用靶点。方法研究对象分为OTCS组(n=15)与正常对照组(n=15)。采用液相色谱-串联质谱法(liquid chromatography-tandem mass spectrometry,LC-MS/MS)技术检测两组患者脑脊液代谢物变化情况,并对差异表达代谢物进行生物信息学分析。采用受试者工作特征(receiver operating characteristic,ROC)曲线验证潜在标志物。结果代谢组学结果显示:与正常对照组比较,OTCS组差异代谢物有87种,其中46种表达上调,41种表达下调。OTCS组患者差异代谢物主要通过TCA循环、GABA神经突触、谷氨酸能突触、氨酰-tRNA生物合成等通路影响疾病发生;其中上调的琥珀酸、L-谷氨酸、L-异亮氨酸及下调的柠檬酸可作为OTCS疾病诊断潜在标志物,经ROC曲线验证具有较好诊断效能。结论代谢组学结果表明:OTCS脑脊液代谢异常,初步将琥珀酸、L-谷氨酸、L-异亮氨酸、柠檬酸作为OTCS疾病诊断潜在标志物。Objective To analyze the metabolic characteristics,metabolic pathways,and action targets of cerebrospinal fluid(CSF)in occult tethered cord syndrome(OTCS).Methods The participants were divided into OTCS group(OTCS patients,n=15)and normal control group(n=15).The changes of CSF metabolites in the two groups were detected by liquid chromatography-tandem mass spectrometry(LC-MS/MS)technology,and the differentially expressed metabolites were analyzed by bioinformatics.And the receiver operating characteristic(ROC)curve was used to verify the potential markers.Results The metabolomics results revealed that compared with the normal control group,there were 87 differentially expressed metabolites in the OTCS group,with 46 being upregulated and 41 downregulated.The differentially expressed metabolites in the OTCS patients mainly affect the occurrence of the disease through the pathways such as the TCA cycle,GABA synapses,glutamatergic synapses,and aminoacyl-tRNA biosynthesis.Among them,the upregulated succinic acid,L-glutamic acid and L-isoleucine,and the downregulated citric acid can be used as potential diagnostic markers for OTCS,demonstrating good diagnostic performance as verified by the ROC curve.Conclusions Metabonomics results indicate that there is abnormal cerebrospinal fluid metabolism in OTCS,and succinic acid,L-glutamic acid,Lisoleucine and citric acid are preliminarily used as potential markers for OTCS diagnosis.
