鲜益母草胶囊治疗经阴道分娩后合并盆底功能障碍产妇对盆底功能康复的效果

摘　　要：目的探讨经会阴三维超声评估经阴道分娩后合并盆底功能障碍(PFD)产妇应用鲜益母草胶囊对盆底功能康复效果的影响。方法回顾性分析2021年9月至2022年7月于杭州师范大学附属医院经阴道分娩的130例产妇的临床资料,根据治疗方式不同分为研究组(口服鲜益母草胶囊联合盆底康复训练)、对照组(盆底康复训练),分别为70、60例,均进行3个月治疗。比较2组康复3个月后子宫脱垂与漏尿情况,静息状态下、缩肛动作时盆膈裂孔面积、左右径、前后径,生活质量及性功能水平与盆底肌力(Ⅰ、Ⅱ类肌)。结果治疗3个月后,研究组子宫脱垂分度(Ⅱ度10例,Ⅰ度19例,无41例)和漏尿产妇比例[22/70(31.4%)]均低于对照组[(Ⅲ度6例,Ⅱ度12例,Ⅰ度24例,无18例),32/60(53.3%)](P<0.05)。治疗3个月后,对照组静息状态下及缩肛动作盆膈裂孔面积(13.0±2.0)cm^(2)、左右径(4.3±1.3)cm、前后径(4.3±1.1)cm均大于研究组[(9.2±1.3)cm^(2)、(3.0±0.7)cm、(3.1±1.0)cm](P均<0.05)。治疗3个月后,研究组女性性功能指标(FSFI)得分(19.2±2.1)分高于对照组(14.2±2.1)分,盆底功能障碍评估问卷(PFDI-20)得分(54.9±10.3)分低于对照组(62.6±10.8)分(P<0.05)。治疗3个月后,研究组Ⅰ、Ⅱ类肌力[(3.59±0.62)、(3.31±0.20)]均大于对照组[(3.02±0.23)、(3.03±0.82)](P<0.05)。结论口服鲜益母草胶囊联合盆底康复训练可改善经阴道分娩后合并盆底功能障碍产妇的盆底肌肉组织形态和功能,有效降低子宫脱垂严重程度,减少漏尿比例及子宫脱垂分度,改善产妇产后生活质量,提高性功能水平,可作为产后盆底功能恢复的治疗措施。Objective To evaluate the effect of motherwort capsule on the rehabilitation of pelvic floor function in parturients with pelvic floor dysfunction(PFD)after vaginal delivery by transperineal three-dimensional ultrasound.Methods A retrospective analysis was conducted on the clinical data of 130 parturients who gave birth through vagina in Hangzhou Normal University Affiliated Hospital from September 2021 to July 2022.They were divided into a study group(oral administration of fresh motherwort capsules combined with pelvic floor reha-bilitation training)and control group(pelvic floor rehabilitation training),with 70 and 60 cases respectively.Both groups were treated for 3 months.The uterine prolapse and urine leakage,the area,left and right diameters,and anterior-posterior diameters of the pelvic hiatus under resting state and during anal contraction,quality of life,sex-ual function,and pelvic floor muscle strength(TypeⅠand TypeⅡmuscles)were compared between the two groups after 3 months of rehabilitation.Results After 3 months of treatment,the uterine prolapse grading in the study group(10 cases of degreeⅡ,19 cases of degreeⅠ,no 41 cases)and the proportion of parturients with urinary leakage(22/70,31.4%)were both lower than those in the control group[6 cases of degreeⅢ,12 cases of degreeⅡ,24 cases of gradeⅠ,18 cases of none,32/60(53.3%)](P<0.05).After 3 months of treatment,(13.0±2.0)cm^(2),left-right diameter(4.3±1.3)cm,and anterior-posterior diameter(4.3±1.1)cm of the control group during rest and anal contraction were all biger than those of the study group[(9.2±1.3)cm^(2),(3.0±0.7)cm,(3.1±1.0)cm](P<0.05),the FSFI score of the study group(19.2±2.1)was higher than that of the control group(14.2±2.1),and the PFDI-20 score(54.9±10.3)was lower than that of the control group(62.6±10.8)(P<0.05).After 3 months of treatment,the muscle strength of TypeⅠandⅡin the study group[(3.59±0.62),(3.31±0.20)]were both greater than those in the control group [(3.02±0.23), (3.03±0.82)] (P<0.05).
