选择性痔上黏膜切除术联合外痔切除术治疗环状混合痔的临床效果

摘　　要：目的 分析选择性痔上黏膜切除术(TST)联合外痔切除术治疗环状混合痔的临床效果。方法 选择我院2021年3月至2023年1月收治的90例环状混合痔患者,根据治疗方案的不同分为TST组(45例)、传统手术组(45例)。传统手术组采用外剥内扎术式治疗,TST组采用TST联合外痔切除术治疗。比较两组患者术后首次排便时及术后3、5、7 d视觉模拟量表(VAS)评分,住院时间,患者满意度,术后尿潴留发生率,术后并发症情况。结果 与传统手术组相比较,TST组患者术后首次排便时间,术后3、5、7 d VAS评分均更低(P <0.05)。TST组住院时间较传统手术组更短(P <0.05)。TST组患者满意度高于传统手术组(P <0.05)。两组患者术后尿潴留发生率相近,差异无统计学意义(P>0.05)。两组患者术后均无肛门狭窄、肛门失禁、肛门坠胀等并发症的发生。结论 TST联合外痔切除术可加快环状混合痔患者术后康复,减轻患者疼痛,患者满意度高。Objective To analyze the clinical effects of tissue-selecting therapystapler(TST) combined with external hemorrhoidectomy in the treatment of circumferential mixed hemorrhoids.Methods A total of 90 patients with circumferential mixed hemorrhoids admitted to our hospital from March 2021 to January 2023 were selected and divided into the TST group(n=45) and the traditional surgery group(n=45) according to different treatment plans.The traditional surgery group was treated with external stripping and internal ligation,while the TST group was treated with TST combined with external hemorrhoidectomy.Postoperative visual analogue scale(VAS) scores during the first defecation and on postoperative days 3,5,and 7,hospitalization time,patient satisfaction,incidence of postoperative urinary retention,and postoperative complications were compared between the two groups.Results Compared with the traditional surgery group,the TST group had significantly lower VAS scores during the first defecation and on postoperative days 3,5,and 7(P<0.05).The hospitalization time in the TST group was significantly shorter than that in the traditional surgery group(P<0.05).Patient satisfaction in the TST group was significantly higher than in the traditional surgery group(P<0.05).There was no significant difference in the incidence of postoperative urinary retention between the two groups(P>0.05).No anal stricture,anal incontinence,or anal bulge occurred in either group postoperatively.Conclusions TST combined with external hemorrhoidectomy can accelerate postoperative recovery in patients with circumferential mixed hemorrhoids,reduce pain,and improve patient satisfaction.
