复方炔诺酮与黄体酮联合治疗对围绝经期异常子宫出血患者的临床疗效

摘　　要：目的探讨复方炔诺酮与黄体酮联合治疗对围绝经期异常子宫出血患者出血情况及血清性激素的影响。方法选择100例围绝经期异常子宫出血患者,均于2021年12月至2023年1月被我院收治,按随机数字表法分两组,各50例。对照组用黄体酮治疗,观察组用复方炔诺酮联合黄体酮治疗。比较两组临床疗效、出血情况、血清性激素水平、子宫内膜厚度和子宫平滑肌收缩幅度。结果观察组临床疗效为94.00%(48/50),高于对照组的80.00%(42/50),有统计学差异(P<0.05)。治疗后,观察组出血量为(113.57±15.21)ml、贫血发生率为4.00%、再次出血率为0,少于对照组的(145.36±18.74)ml、20.00%、12.00%;止血时间为(5.58±2.13)h,短于对照组的(8.53±2.41)h;卵泡刺激素(FSH)水平为(33.97±2.23)U/L、黄体生成素(LH)水平为(27.64±1.75)U/L,低于对照组的(40.78±2.35)U/L、(32.56±1.98)U/L;雌二醇(E_(2))水平为(40.15±1.49)pmol/L,高于对照组的(34.08±1.31)pmol/L;子宫内膜厚度为(4.31±0.74)mm,低于对照组的(5.26±0.85)mm;子宫平滑肌收缩幅度为(9.34±1.25)g,高于对照组的(8.05±1.12)g,均有统计学差异(P<0.05)。结论复方炔诺酮联合黄体酮对围绝经期异常子宫出血患者有明显的止血效果,可有效改善患子宫内膜厚度,调节患者性激素水平,安全可靠。Objective To explore the effect of combined treatment of compound norethisterone and progesterone on bleeding and serum sex hormones in patients with abnormal uterine bleeding during perimenopause.Method A total of 100 cases of abnormal uterine bleeding in perimenopausal period were selected,they were admitted to our hospital from December 2021 to January 2023.They were randomly divided into 2 groups with 50 cases each.The control group was treated with progesterone,while the observation group was treated with compound norethisterone combined with progesterone.Compare the two groups' clinical efficacy,bleeding status,serum sex hormone levels,endometrial thickness,and uterine smooth muscle contraction amplitude.Result The clinical efficacy of the observation group was 94.00%(48/50),which was higher than the 80.00%(42/50) of the control group,with a statistical difference(P<0.05).After treatment,the observation group had a bleeding volume of(113.57±15.21) ml,an anemia incidence rate of 4.00%,and a rebleeding rate of 0,which was lower than the control group's(145.36±18.74) ml,20.00%,and 12.00%.The hemostatic time was(5.58±2.13) hours,shorter than the control group's(8.53±2.41) hours.The levels of follicle stimulating hormone(FSH) were(33.97±2.23) U/L,luteinizing hormone(LH) were(27.64±1.75) U/L,and lower than the control group's(40.78±2.35) U/L,(32.56 ±1.98) U/L,estradiol(E_(2)) level was(40.15±1.49) pmol/L,higher than the control group's(34.08±1.31) pmol/L,endometrial thickness was(4.31±0.74) mm,lower than the control group's(5.26±0.85) mm,uterine smooth muscle contraction amplitude was(9.34±1.25) g,higher than the control group's(8.05±1.12) g,with a statistical difference(P<0.05).Conclusions Compound Norethisterone combined with progesterone has obvious hemostatic effect on patients with abnormal uterine bleeding in perimenopausal period,can effectively improve the endometrial thickness,adjust the level of sex hormone in patients,and is safe and reliable.
