摘　　要：目的 探究全科医生管理对社区老年慢性病患者疾病知晓率、自我管理能力的提升效果。方法 选取在我院就诊的社区老年病患者100例,随机均分为对照组和观察组,每组50例。对照组进行常规社区管理,观察组进行全科医生管理。统计两组的疾病知晓率、自我管理能力、健康素养。结果 观察组患者的疾病知晓率、自我管理能力、健康素养评分均优于对照组(P <0.05)。结论 对于社区老年慢性病患者实施全科医生管理,可有效提升患者疾病知晓率以及健康素养,加强患者自我管理。Objective To explore the improvement effect of general practitioner management on the disease awareness rate and self-management ability of elderly chronic disease patients in the community.Methods A total of 100 community geriatric patients in our hospital were selected and randomly divided into control group and observation group,with 50 cases in each group.The control group underwent routine community management,and the observation group performed the general practitioner management.Disease awareness rate,self-management ability and health literacy of the two groups were counted.Results Disease awareness,self-management ability,and health literacy score were better than that of the control group(P<0.05).Conclusions The implementation of general practitioner management for the elderly patients with chronic diseases in the community can effectively improve the disease awareness rate and health literacy of patients,and strengthen the self-management of patients.
