加味二陈汤治疗小儿痰湿蕴肺型肺炎喘嗽临床观察

摘　　要：目的:观察加味二陈汤治疗小儿痰湿蕴肺型肺炎喘嗽的临床疗效。方法:选取104例符合纳入标准的肺炎喘嗽患儿,随机分为两组各52例,对照组予常规西医治疗,治疗组在对照组基础上予加味二陈汤治疗,比较两组患儿的临床疗效、SGRQ评分、MRC评分、呼吸功能及炎症反应。结果:治疗组总有效率96.15%,对照组总有效率80.77%,两组比较差异有统计学意义(P﹤0.05)。治疗后,治疗组SGRQ评分、MRC评分及白介素-4(IL-4)、白介素-6(IL-6)、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)水平明显低于对照组(P﹤0.05);治疗组一秒用力呼气容积(FEV1)、用力呼气量(FVC)、最大呼气流速(PEF)、每分钟最大通气量(MVV)均明显高于对照组(P﹤0.01)。结论:加味二陈汤治疗痰湿蕴肺型肺炎喘嗽患儿疗效较好,可改善患儿症状及体征。Objective:To observe the clinical efficacy of modified Erchen decoction on children's pneumonia-caused asthma and cough with syndrome of accumulation of phlegm-dampness in the lung.Methods:104 cases of eligible children patients with pneumonia-caused asthma and cough were selected and randomly divided into two groups,52 cases in each group.The control group was treated with conventional western medicine,while the treatment group was treated plus with modified Erchen decoction on the basis of control group.Clinical efficacy,SGRQ score,MRC score,respiratory function and inflammatory reaction of children patients were compared between two groups.Results:The total effective rate was 96.15%in the treatment group and 80.77%in the control group,and the comparative difference between two groups was statistically significant(P﹤0.01).After treatment,SGRQ score,MRC score and the levels of interleukin-4(IL-4),interleukin-6(IL-6),C-reactive protein(CRP)and tumor necrosis factor-α(TNF-α)of treatment group were significantly lower than those of control group(P﹤0.01);forced expiratory volume in one second(FEV1),forced vital capacity(FVC),peak expiratory flow(PEF)and minute ventilation volume(MVV)of treatment group were significantly higher than those of control group(P﹤0.01).Conclusion:The clinical efficacy of modified Erchen decoction on children's pneumonia-caused asthma and cough with syndrome of accumulation of phlegm-dampness in the lung is better,which can improve the symptoms and body signs of children patients.
