摘 要:目的探究血清降钙素原(PCT)、C反应蛋白(CRP)在不同发病日龄新生儿败血症中诊断价值。方法选取2020年1月至2022年8月抚州市第一人民医院收治的60例败血症新生儿作为研究对象,以不同发病时间分为早发型组与晚发型组,各30例。所有患儿均行血清PCT、CRP检测,比较两组PCT、CRP水平,分析PCT、CRP检测及联合检测对新生儿败血症的诊断效能。结果晚发型组CRP、PCT水平均高于早发型组,差异有统计学意义(P<0.05)。PCT、CRP检测诊断新生儿败血症的截断值分别为6.21 mg/L、4.76μg/L,CRP联合PCT诊断新生儿败血症的AUC、灵敏度、特异度最高,分别为0.942、90.00%、93.33%,诊断效能最高。结论PCT、CRP诊断新生儿败血症的截断值分别为6.21mg/L、4.76μg/L,两者联合检测可提升对不同发病日龄新生儿败血症的诊断效能。Objective To explore the diagnostic value of serum procalcitonin(PCT)and C-reactive protein(CRP)in neonatal septicemia at different onset days.Methods 60 neonates with septicemia treated in Fuzhou First People's Hospital from January 2020 to August 2022 were selected as the study subjects,they were divided into the early-onset group and the late-onset group according to the different onset time,with 30 cases in each group.Serum PCT and CRP were detected in all the children,the levels of PCT and CRP were compared between the two groups,and the diagnostic efficacy of PCT and CRP and combined detection in neonatal septicemia was analyzed.Results The level of CRP,PCT in the late-onset group was higher than that in the early-onset group,and the differences were statistically significant(P<0.05).The Cutoff values of PCT and CRP detection in diagnosis of neonatal sepsis were 6.21 mg/L and 4.76μg/L,respectively.The AUC,sensitivity and specificity of CRP combined with PCT in the diagnosis of neonatal sepsis were the highest,which 0.942,90.00%and 93.33%,respectively,and the diagnostic efficacy was the highest.Conclusion The cutoff values of PCT and CRP in diagnosis of neonatal sepsis are 6.21 mg/L and 4.76μg/L,respectively,the combined detection of PCT and CRP could improve the diagnostic efficacy of neonatal sepsis at different onset ages.
