摘　　要：目的探讨超声引导下股神经及坐骨神经阻滞麻醉在老年下肢骨折手术中的应用效果。方法选取医院2021年7月至2023年6月收治的老年下肢骨折患者130例,按照随机数字表法分为对照组和观察组,各65例。对照组患者予腰硬联合麻醉,观察组患者予超声引导下股神经及坐骨神经阻滞麻醉。结果观察组患者的阻滞起效时间显著短于对照组,阻滞维持时间显著长于对照组;拔管后30 min,视觉模拟评分量表评分显著低于对照组;自主呼吸恢复时间、拔管时间及完全恢复时间均显著短于对照组(P<0.05)。观察组患者的并发症发生率为4.62%,显著低于对照组的15.38%(P<0.05)。结论超声引导下股神经及坐骨神经阻滞麻醉应用于老年下肢骨折手术,麻醉效果显著,术后恢复快,并发症少。Objective To investigate the application effect of ultrasound-guided femoral and sciatic nerve block anesthesia in elderly lower limb fracture surgery.Methods A total of 130 elderly patients with lower limb fractures admitted to the hospital from July 2021 to June 2023 were selected and randomly divided into the control group and the observation group by the random number table method,with 65 cases in each group.The patients in the control group were given combined spinal and epidural anesthesia,while the patients in the observation group were given ultrasound-guided femoral and sciatic nerve block anesthesia.Results The onset time of block in the observation group was significantly shorter than that in the control group,and the maintenance time of block was significantly longer than that in the control group;after 30 min of extubation,the Visual Analogue Scale score was significantly lower than that in the control group;the recovery time of spontaneous respiration,extubation time,and complete recovery time was significantly shorter than that in the control group(P<0.05).The incidence of complications in the observation group was 4.62%,which was significantly lower than 15.38% in the control group(P<0.05).Conclusion Ultrasound-guided femoral and sciatic nerve block anesthesia applied in elderly lower limb fracture surgery is effective,and the patients recover quickly after surgery with fewer complications.
