摘 要:目的分析弥散张量成像(DTI)技术用于评估腰椎间盘突出症患者经皮椎间孔镜髓核摘除术(PTED)后受压神经根微观结构改善的效果。方法选取医院2022年3月至2023年3月收治的行PTED治疗的腰椎间盘突出症患者53例,手术前后为患者进行DTI扫描,比较神经根受压侧与正常侧的各向异性分数值(FA)及表面弥散系数值(ADC)。结果术前,神经根受压侧FA显著低于正常侧(P<0.05);术后,FA受压侧显著高于术前(P<0.05),ADC受压侧显著低于术前(P<0.05),但与正常侧比较均无显著差异(P>0.05)。结论应用DTI评估PTED后腰椎间盘突出症患者的受压神经根的微观结构改善情况,可获得良好的评估效果。Objective To analyze the effect of diffusion tensor imaging(DTI)technique for assessing the improvement of the compressed nerve root microstructure after percutaneous translaminar endoscopic discectomy(PTED)in patients with lumbar disc herniation.Methods Fifty-three patients with lumbar disc herniation treated with PTED admitted to the hospital from March 2022 to March 2023 were selected,DTI scans were performed before and after the operation,and the fractional anisotropy(FA)and apparent diffusion coefficient(ADC)of the compressed side of the nerve root and the normal side were compared.Results The FA on the nerve root compressed side was significantly lower than that on the normal side before surgery(P<0.05).After surgery,the FA on the nerve root compressed side was significantly higher than that before surgery(P<0.05),and the ADC on the compressed side was significantly lower than that before surgery(P<0.05),but there was no significant difference when compared with that on the normal side(P>0.05).Conclusion The application of DTI to evaluate the microstructure improvement of compressed nerve roots in patients with lumbar disc herniation after costal PTED is effective.
