摘　　要：目的:研究慢性阻塞性肺疾病(COPD)合并睡眠呼吸障碍患者认知功能障碍(CI)的相关影响因素。方法:选取兴义市人民医院2022年9月~2023年2月收治的40例COPD合并睡眠呼吸障碍患者为研究对象,采集受试者吸烟、肺功能、睡眠监测、血气分析、血常规、C反应蛋白、蒙特利尔认知评估量表(MoCA)等数据。按照CI情况分为非CI组(n=16)与CI组(n=24),对CI的相关影响因素进行探讨。结果:与非CI组患者比较,CI组吸烟、COPD病程≥5年、低氧血症、肺功能重度损害、AHI指数及炎性反应负荷指数的比例更高,差异有统计学意义(P<0.05)。结论:吸烟、COPD病程≥5年、低氧血症、肺功能重度损害、AHI指数及炎性反应负荷指数可能是COPD合并睡眠呼吸障碍患者发生认知功能障碍的高危因素。Objective To explore the related factors of cognitive impairment(CI)in patients with chronic obstructive pulmonary disease(COPD)and sleep apnea.Method 40 patients with COPD and sleep apnea in our hospital from September 2022 to February 2023 were selected as research subjects,and smoking,lung function,sleep monitoring,blood gas analysis,blood routine,C-reactive protein,and Montreal Cognitive Assessment Scale(MoCA)were collected.According to the situation of CI,they were divided into 16 cases of non-CI group and 24 cases of CI group,and the relevant influencing factors of CI were discussed.Results Compared with the non-CI group,the proportion of smoking,COPD duration≥5 years,hypoxemia,severe impairment of lung function,AHI index and inflammatory coincidence index in CI group was higher(P<0.05).Conclusion Smoking,COPD duration≥5 years,hypoxemia,severe impairment of lung function,AHI index,and inflammatory load index may be high risk factors for cognitive dysfunction in patients with COPD and sleep apnea.
