摘　　要：目的探索经腹壁瘢痕入路单孔腹腔镜手术治疗妇科疾病的可行性及安全性。方法回顾性分析2019年8月至2024年1月在南京医科大学附属常州第二人民医院妇科行经腹壁瘢痕入路单孔腹腔镜手术的26例患者的临床资料,对其术中、术后情况进行总结分析。结果26例经腹壁瘢痕入路单孔腹腔镜手术均顺利完成,术中无一例更改手术入路,术后未出现严重并发症,恢复良好。手术时间为50~165(8885±3718)min,术中出血量为1000(1000,4500)mL,血红蛋白下降值为1050(600,1650)g/L,术后住院时间为3~8(569±123)d,住院总费用为16675~32802(2306863±419900)元。结论经腹壁瘢痕入路单孔腹腔镜手术治疗特定患者的特定妇科疾病是安全可行的,有较好的微创和美容优势。Objective To explore the feasibility and safety of single-port laparoscopic surgery via abdominal scar approach in treatment of gynecologic disorders.Methods The clinical data of 26 patients who underwent single-port laparoscopic surgery via abdominal scar approach in Department of Gynecology,Changzhou No.2 People's Hospital,the Affiliated Hospital of Nanjing Medical University from August 2019 to January 2024 were retrospectively analyzed,and the patients'intraoperative situation and postoperative situation were summarized and analyzed.Results The gynecologic operations performed by using single-port laparoscopy via abdominal scar approach were successfully completed in all the 26 cases.No surgical approaches were changed during the operations in the patients,and no serious complications occurred after the operations,and the patients recovered well.The operative time was 50-165(88.85±37.18)minutes,and the intraoperative blood loss was 10.00(10.00,45.00)milliliters,and the value of the decrease in hemoglobin was 10.50(6.00,16.50)g/L,and the postoperative hospitalization time was 3-8(5.69±1.23)days,and the total hospitalization costs were 16675-32802(23068.63±4199.00)Chinese Yuan.Conclusion Single-port laparoscopic surgery via abdominal scar approach is safe and feasible to treat some specific gynecologic disorders in some specific patients,and has relatively good minimally invasive and cosmetic advantages.
