补肾活血方联合富血小板血浆于关节镜下治疗肩袖损伤疗效分析

摘　　要：目的:观察补肾活血方联合富血小板血浆于关节镜下治疗肩袖损伤的临床疗效。方法:选取符合标准的60例肩袖损伤患者,其中男25例,女35例,年龄为33~70(51.38±8.84)岁,病程时间为月0.5~8(3.70±1.95)个月,随机分为3组,每组各20例,A组采用单纯关节镜修补术,B组采用关节镜修补术+PRP注射,C组采用关节镜下修补+PRP注射+补肾活血方口服,记录并比较3组患者术前、术后2月及术后6月的Constant-Murley评分、NEER评分、牛津大学肩关节评分(OSS评分),以及肩关节主动前屈关节活动度、主动外展关节活动度等。结果:60例患者术后均未发生感染,手术时间为50~80min(64.08±7.19)min,出血量为(11.70±5.49)mL;术后随访时间为6~12个月(8.88±2.04)月;术中检查肩袖组织均固定牢靠,活动无撞击,3组患者术后2月、6月的Constant-Murley评分及NEER评分均较术前上升,且C组分数最高,各组间比较差异具有统计学意义(P<0.05);3组患者OSS评分术后2月、6月较术前明显降低,且C组分数最低,各组间比较差异具有统计学意义(P<0.05);术后2月、6月AFROM及AAROM均不同幅度提升,各组间比较差异具有统计学意义(P<0.05)。结论:补肾活血血方联合富血小板血浆于关节镜下治疗肩袖损伤可以有效缓解患者疼痛,改善肩关节活动度,促进肩关节功能恢复。Objective:To observe the clinical efficacy of Bushen Huoxue Recipe combined with platelet-rich plasma in the treatment of rotator cuff injury under arthroscopy.Methods:60 patients with rotator cuff injury who met the cri-teria were selected,including 25 males and 35 females,aged 33~70(51.38±8.84)years,and the disease duration was 0.5~8(3.70±1.95)months,randomly.Divided into 3 groups,each group has 20 cases.Group A uses simple arthro-scopic repair,group B uses arthroscopic repair+PRP injection,and group C uses arthroscopic repair+PRP injection+oral administration of Bushen Huoxue Recipe,recorded and compared.Constant-Murley score,NEER score,Oxford University Shoulder Score(OSS score),active forward flexion range of motion,and active abduction range of motion of the shoulder joint in the three groups of patients before surgery,2 months after surgery,and 6 months after surgery wait.Results:None of the 60 patients suffered from postoperative infection,the operation time was 50~80min(64.08±7.19)min,and the bleeding volume was(11.70±5.49)ml;the postoperative follow-up time was 6~12 months(8.88±2.04)Months;intraoperative examination showed that the rotator cuff tissue was firmly fixed and there was no impact during movement.The Constant-Murley score and NEER score of the three groups of patients at 2 months and 6 months after surgery were all higher than those before surgery,and group C had the highest score.Comparison between groups The difference was statistically significant(P<0.05);the OSS scores of the three groups of patients at 2 and 6 months after surgery were significantly lower than those before surgery,and group C had the lowest score.The difference between the groups was statistically significant(P<0.05);AFROM and AAROM increased to varying degrees at 2 months and 6 months after surgery,and the differences between each group were statistically significant(P<0.05).Conclusion:Bush-en Huoxuexue Recipe combined with platelet-rich plasma can effectively relieve patients’pain,improve shoulder joint mobility
