度普利尤单抗治疗特应性皮炎致面部传染性软疣1例

摘　　要：61岁女性患者。全身红斑20余年,左侧面颊丘疹2周。3个月前患者因全身特应性皮炎反复发作20余年接受度普利尤单抗治疗,2周前左侧面颊部多发绿豆至黄豆大小肤色丘疹,其中颞部可见一花生米大小红色结节。结节组织病理示:真皮内可见呈分叶、内生性生长的结节状病灶,病灶内细胞的胞质有嗜酸性包涵体。诊断:传染性软疣;特应性皮炎(中重度)。治疗:外用院内制剂祛疣散外敷治疗疣体,并继续进行度普利尤单抗治疗特应性皮炎。A 61-year-old female patient presented with a history of generalized erythema for over 20 years and recent onset of papules on the left cheek within the past two weeks.The dupilumab treatment was initiated three months ago due to the persistence of recurrent rashes for over two decades.Two weeks ago,the left cheek exhibited multiple skin-colored papules ranging in size from mung beans to soybeans A red nodule,approximately the size of a peanut,was noted in the temporal region.Histopathological examination of the nodular lesion revealed a nodule of the dermis exhibiting lobulated appearance and endogenous growth,as well as cells containing eosinophilic inclusions within the lesion.The patient was diagnosed with moderate-to-severe atopic dermatitis and molluscum contagiosum.The treatment involved excision of the nodule in the left temporal region for biopsy,topical application of a wart removal preparation to the remaining lesions,and continuation of dupilumab therapy.
