摘 要:视网膜动脉阻塞(RAO)是猝然一眼或双眼视力急剧下降,以视网膜可见典型的缺血性改变为特征的严重致盲性眼病。中医学理论认为络脉循行与眼病的生理与病理关系密切。本文基于络脉理论探讨RAO的治疗,为运用补气行血通络法治疗本病提供理论基础。临床上,该病患者通常为中老年龄,并多伴有高血压病、高脂血症等全身疾病,气虚血瘀是本病病机的关键。若阳气亏虚,兼有气滞、血瘀,外邪因素阻于络脉,气血运行不畅,瘀血阻络则可导致RAO发生。针对上述病机,治疗上以补气行血为本,配合使用通络行滞等治法,往往可收获较好疗效。Retinal artery occlusion(RAO)is a severe blinding eye disease characterized by a sudden,dramatic decrease in vision in one or both eyes,accompanied by typical ischemic changes visible in the retina.Traditional Chinese Medicine(TCM)theory posits that the collateral vessels play a significant role in the physiology and pathology of eye diseases.This paper explores the treatment of RAO based on collateral vessel theory,providing a theoretical foundation for using Qi supplementation,blood circulation,and collateral unblocking therapy to treat this condition.Clinically,patients with RAO are typically middle-aged or elderly and often suffer from systemic diseases such as hypertension and hyperlipidemia.Qi deficiency and blood stasis are key pathomechanisms.When Yang Qi is deficient,accompanied by Qi stagnation and blood stasis,external pathogenic factors obstruct the collateral vessels,leading to impaired Qi and blood flow,which can result in RAO.Addressing the aforementioned pathomechanisms,the primary treatment involves Qi supplementation and blood circulation,combined with therapies that unblock the collaterals and resolve stagnation,often yielding favorable outcomes.
