密盖息联合克氏针固定治疗骨质疏松性桡骨远端骨折的临床疗效及对愈合时间的影响

摘　　要：目的分析鲑鱼降钙素注射液(商品名:密盖息)联合克氏针固定治疗骨质疏松性桡骨远端骨折的临床疗效和对愈合时间的影响。方法80例骨质疏松性桡骨远端骨折患者,通过随机数字抽签方式分为对照组和观察组,各40例。对照组进行克氏针固定治疗,观察组在此基础上联合密盖息治疗。对比两组患者治疗前后Barthel指数、视觉模拟评分法(VAS)评分、临床疗效、关节恢复情况。结果两组患者治疗前的Barthel指数、VAS评分对比差异不存在统计学意义(P>0.05);治疗后,观察组的Barthel指数(83.27±7.37)分高于对照组的(72.74±6.16)分,对比差异存在统计学意义(P<0.05);观察组治疗后1周以及治疗后3个月的VAS评分分别为(2.49±0.31)、(1.02±0.23)分,均较对照组的(3.36±0.42)、(2.38±0.35)分低,对比差异存在统计学意义(P<0.05)。治疗后,与对照组的77.50%相比,观察组总有效率95.00%更高,对比差异存在统计学意义(χ^(2)=5.165,P<0.05)。观察组治疗后的骨密2度(0.72±0.26)g/cm大于对照组的(0.61±0.22)g/cm2,且骨折愈合时间(30.31±4.72)d与桡骨茎突和尺骨茎突远端间距(0.68±0.18)cm短于对照组的(44.39±5.63)d、(1.16±0.24)cm,对比差异存在统计学意义(P<0.05);治疗前组间的骨密度对比差异无统计学意义(P>0.05)。结论在骨质疏松性桡骨远端骨折的治疗中应用密盖息联合克氏针固定,能减轻患者的疼痛感,缩短骨愈合时间,促进机体恢复。Objective To analyze the clinical efficacy of Salmon calcitonin injection(trade name:Miacalcic)combined with Kirschner's wire fixation in the treatment of osteoporotic distal radius fracture and its influence on healing time.Methods A total of 80 patients with osteopolitic distal radius fracture were divided into a control group and an observation group by random number drawing method,with 40 cases in each group.The control group was treated with Kirschner's wire fixation,and the observation group was combined with Miacalcic on this basis.Barthel index and visual analogue scale(VAS)score before and after treatment,clinical efficacy,and joint recovery were compared between two groups.Methods Before treatment,there was no significant difference in Barthel index and VAS score between the two groups(P>0.05).After treatment,the observation group had higher Barthel index of(83.27±7.37)points than(72.74±6.16)points in the control group,and the difference was statistically significant(P<0.05).VAS scores of the observation group at 1 week and 3 months after treatment were(2.49±0.31)and(1.02±0.23)points,which were lower than(3.36±0.42)and(2.38±0.35)points of the control group,and the difference was statistically significant(P<0.05).After treatment,the total effective rate of the observation group was 95.00%,which higher than that of the control group(77.50%),and the difference was statistically significant(χ^(2)=5.165,P<0.05).After treatment,the observation group had 22 higher bone mineral density of(0.72±0.26)g/cm than(0.61±0.22)g/cm in the control group;the observation group had fracture healing time of(30.31±4.72)d and the distance between the distal styloid of the radius and ulna of(0.68±0.18)cm,which were shorter than(44.39±5.63)d and(1.16±0.24)cm in the control group;the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in bone mineral density between groups(P>0.05).Conclusion In the treatment of osteoporotic distal radius fracture,the combination of Mi
