关节镜下保留与不保留残端在前交叉韧带重建术治疗中的应用对照分析

摘　　要：目的比较关节镜下保留与不保留残端在前交叉韧带重建术治疗中的应用效果。方法选取62例前交叉韧带断裂患者,使用随机数字表法分为研究组(n=31)与对照组(n=31)。两组患者均行关节镜前交叉韧带重建术,其中研究组术中保留残端,对照组术中不保留残端。比较两组Lysholm膝关节评分、患侧膝关节位置觉(JPS)以及术后并发症发生情况。结果术前,研究组Lysholm膝关节评分、JPS值分别为(60.00±4.56)分、(4.95±1.25)°,对照组分别为(60.12±5.74)分、(4.96±1.34)°;术后6个月,研究组Lysholm膝关节评分、JPS值分别为(90.50±4.05)分、(2.89±0.68)°,对照组分别为(84.85±4.40)分、(3.50±0.70)°。术后6个月,两组Lysholm膝关节评分、JPS值与术前比较均改善,其中研究组Lysholm膝关节评分高于对照组,JPS值低于对照组(P<0.05)。术后并发症发生率比较中,两组结果未见差异性(P>0.05)。结论相较于关节镜前交叉韧带重建术中不保留残端,保留残端可以有效改善前交叉韧带断裂患者的膝关节功能,促进本体感觉恢复,且未增加术后并发症风险,具有临床应用价值。Objective To compare the practical effect of preservation and non preservation of stumps in arthroscopic anterior cruciate ligament reconstruction.Methods 62 patients with anterior cruciate ligament rupture were divided into a study group(n=31)and a control group(n=31)according to random number table method.Both groups underwent arthroscopic anterior cruciate ligament reconstruction,with the study group preserving the stump during surgery and the control group not preserving the stump during surgery.The Lysholm knee score,joint position sense(JPS)and postoperative complications were compared between the two groups.Results Before surgery,the Lysholm knee score and JPS values in the study group were(60.00±4.56)points and(4.95±1.25)°,and those in the control group were(60.12±5.74)points and(4.96±1.34)°.6 months after surgery,the Lysholm knee score and JPS values in the study group were(90.50±4.05)points and(2.89±0.68)°,and those in the control group were(84.85±4.40)points and(3.50±0.70)°.6 months after surgery,the Lysholm knee score and JPS value in both groups were improved compared with those before surgery;the Lysholm knee score in the study group was higher than that in the control group,and JPS value was lower than that in the control group(P<0.05).There was no difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusion Preservation of stumps during arthroscopic anterior cruciate ligament reconstruction can effectively improve knee joint function of patients with anterior cruciate ligament rupture,promote proprioceptive recovery,and do not increase the risk of postoperative complications.It has clinical application value.
