床旁实时超声与传统盲插导管在危重症新生儿脐静脉置管中应用对比

摘　　要：目的探究床旁实时超声与传统盲插导管在危重症新生儿脐静脉置管(UVC)中应用效果。方法选择2022年1月至2023年9月在周口市中心医院需UVC危重症新生儿100例,按照治疗方法不同将其分为对照组(传统盲插导管)及研究组(床旁实时超声)各50例,对比两组一次性插管成功率、留置时间、插管所用时间、插管固定次数、炎症因子、并发症。结果研究组一次性插管成功率、留置时间高于对照组,差异有统计学意义(P<0.05)。研究组插管所用时间、插管固定次数低于对照组,差异有统计学意义(P<0.05)。研究组C反应蛋白(CRP)、降钙素原(PCT)低于对照组,差异有统计学意义(P<0.05)。研究组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论床旁实时超声辅助UVC一次性置管成功率更高,且可减轻炎症反应,减少并发症发生情况,置管留置时间较长。Objective To explore the application effect of bedside real-time ultrasound and traditional blind catheterization in umbilical venous catheterization(UVC)in critically ill neonates.Methods A total of 100 critically ill neonates who needed UVC in Zhoukou Central Hospital from January 2022 to September 2023 were selected.According to different treatment methods,they were divided into the control group(traditional blind catheterization)and the research group(bedside real-time ultrasound),each with 50 cases.The one-time catheterization success rate,retention time,catheterization time,number of catheter fixations,inflammatory factors,and complications of the two groups were compared.Results The one-time catheterization success rate and retention time of the research group were higher than those of the control group,with statistically significant differences(P<0.05).The catheterization time and the number of catheter fixations in the research group were lower than those in the control group,with statistically significant differences(P<0.05).The C-reactive protein(CRP)and procalcitonin(PCT)in the research group were lower than those in the control group,with statistically significant differences(P<0.05).The incidence of complications in the research group was lower than that in the control group,with statistically significant differences(P<0.05).Conclusion The one-time catheterization success rate of UVC assisted by bedside real-time ultrasound is higher,and it can reduce the inflammatory response and the occurrence of complications,and the catheter retention time is longer.
