摘 要:目的:探讨气管内清吸拔管技术在低出生体重儿机械通气中的应用效果。方法:选取2022年8月至2023年8月新入我院80例需行有创呼吸机辅助通气的低出生体重儿为研究对象,随机分为对照组和观察组,每组40例。对照组患儿给予常规气管插管集束化护理,气管导管拔管时采用常规拔管方式;观察组在对照组基础上采用气管内清吸拔管方式。比较2组患儿气管导管拔管后血氧饱和度(SPO2)、呼吸(R)、心率(HR)变化,拔管时及拔管后吸出痰块次数,重复插管次数。结果:气管导管拔管后,观察组SPO2更高(P<0.05),R、HR更平稳(P<0.05),气管导管拔管时及拔管后观察组吸出痰块次数均多于对照组(P<0.05);观察组重复插管次数少于对照组(P<0.05)。结论:气管内清吸拔管技术可有效清除呼吸道分泌物,减少患儿气管导管拔管后重复插管次数,促进患儿顺利撤除有创呼吸机,有利于减轻患儿痛苦。Objective:To explore the application effect of endotracheal suction and extubation technology in low birth weight infants with mechanical ventilation.Methods:From August 2022 to August 2023,80 low birth weight infants who needed invasive mechanical ventilation in our hospital were selected as the research objects.They were randomly divided into control group and observation group,with 40 cases in each group.The control group was given conventional tracheal intubation nursing bundle,and the conventional extubation method was used when the tracheal tube was extubated.The observation group was treated with endotracheal suction and extubation on the basis of routine nursing of the control group.The general data of the two groups were compared,and the changes of blood oxygen saturation,respiration and heart rate after extubation,the times of sputum block suction during extubation,the times of repeated intubation,and the times of sputum block suction after weaning were observed and recorded.Results:After extubation,SPO2 was higher(P<0.05),R and HR were more stable(P<0.05)in the observation group,and the number of sputum aspiration in the observation group was significantly more than that in the control group(P<0.05).The number of repeated intubation in the observation group was significantly lower than that in the control group(P<0.05).After weaning,the observation group had significantly more times of sputum aspiration than the control group(P<0.05).Conclusion:Endotracheal suction and extubation technology can effectively remove large particles of respiratory secretions,reduce the number of repeated intubation after tracheal tube extubation,promote the successful removal of invasive ventilator in children,and help reduce the pain of children.
