摘  要:目的探讨早孕期彩色多普勒超声检测子宫动脉频谱在预测妊娠合并抗磷脂抗体综合征(APS)妊娠结局中的价值。方法选取本院收治的妊娠合并APS患者115例临床及超声检查资料为观察组,选取同期在本院就诊的临床及超声检查资料完整的健康孕妇100例为对照组。所有孕妇均接受早孕期子宫动脉超声检查。统计分析两组间妊娠早期子宫动脉RI、S/D、PI以及不良妊娠发生率,分析子宫动脉超声检查判定妊娠合并APS妊娠结局的效能。结果观察组孕妇子宫动脉血流参数PI、RI、S/D均>对照组,差异均有统计学意义(P<0.05)。观察组子宫动脉频谱异常发生率及不良妊娠发生率均高于对照组。结论孕早期子宫动脉彩色多普勒超声血流参数可预测妊娠合并APS结局的价值,并为临床及早诊疗提供客观的参考依据。Objective To explore the predictive value of the uterine artery Doppler in first trimester for poor prognosis in pregnant women with antiphospholipid antibody syndrome.Methods The clinical data of 115 pregnant women with antiphospholipid antibody syndrome treated in Zaozhuang municipal hospital were retrospectively analyzed and set as the case group.The clinical data of 100 healthy pregnant women were analyzed in the same period and set as the healthy group.The uterine artery RI,PI and S/D in the first trimester of pregnancy were compared between the two groups.Result RI,PI,and S/D of uterine artery in early pregnancy of case group were higher than those of healthy group.The differences were statistically significant(P<0.05).The incidence rate of abnormal flow waveform of uterine artery of case group was higher than that of healthy group,as well as adverse pregnancy outcome.Conclusion Uterine artery ultrasound in the first trimester of pregnancy can provide scientific reference data for the treatment of pregnancy with antiphospholipid antibody syndrome and affectively evaluate the risk of adverse pregnancy.
