摘 要:目的探讨应用T_(2)mapping量化比较急性前交叉韧带(ACL)损伤患者患侧与正常侧关节软骨基质的变化。方法选取急性及亚急性ACL损伤患者(创伤后平均32天)30例,患者均进行双侧膝关节MRI检查,包括常规MRI检查及T_(2)mapping序列,之后对胫股关节不同部位深浅层关节软骨进行测量T_(2)值及关节软骨厚度,运用配对t检验评估患侧及正常关节软骨的差异。结果与正常膝关节相比,患侧外侧缘浅表股骨中央前部、股骨后缘后部,外侧缘深层股骨后缘后部,内侧缘浅表股骨前缘后部,内侧缘深层股骨中央后部、胫骨前部、胫骨中央、胫骨后部等部位患侧关节软骨T_(2)值明显较高,差异具有统计学意义(P<0.05)。患侧内外侧缘深浅层关节软骨T_(2)值相比,除了外侧缘股骨后缘后部及内侧缘股骨前缘后部外,其余所有部位浅层T_(2)值较深层明显较高,差异具有统计学意义(P<0.05)。外侧缘浅表部位股骨前缘前部及内侧缘深层部位胫骨前部,患侧关节软骨T_(2)值较正常侧低,差异具有统计学意义(P<0.05)。内侧缘股骨前缘后部、胫骨后部等部位患侧关节软骨厚度较正常侧薄,差异具有统计学意义(P<0.05)。结论ACL损伤患者患侧关节软骨多处T_(2)弛豫时间较对侧升高并且多处关节软骨变薄。ACL损伤是PTOA发生发展的重要原因。Objective To conduct a quantitative comparison of changes in the articular cartilage matrix between the affected and normal sides in patients with acute ACL injury using T_(2) mapping.Methods Thirty patients with acute and subacute ACL injuries(average 32 days after trauma)were collected,and all patients underwent bilateral knee MRI examinations,including conventional MRI examinations and T_(2)mapping sequences,followed by measurement of deep and superficial articular cartilage in different parts of the tibiofemoral joint.T_(2) values and articular cartilage thickness were measured,and the differences between the affected and normal articular cartilage were evaluated using paired t-tests.Results Compared with the normal side,T_(2) values were higher in aCF,pPFlateral superficial zone,pPF in the deep zone,and pTr in the medial superficial zone(P<0.05).The T_(2) value of the articular cartilage on the affected side of aTr at the superficial part of the lateral side and AT in medial deep zone was higher than that on the contralateral side(P<0.05).Compared to the superficial and deep articular cartilage T_(2) values at the affected medial and lateral side,the superficial articular cartilage T_(2) values were significantly higher than the deep ones at all sites except for the lateral pPF and medial pTr(P<0.05).The thickness of pTr,PT articular cartilage on the affected side of the medial side was lower than that on the normal side.All these differences were statistically significant(P<0.05).Conclusion Patients with ACL injury have increased T_(2) relaxation time and thinning of articular cartilage on several sites on the affected side compared to the contralateral side.ACL injury is an important cause of the development of PTOA.
