摘　　要：目的对比胃大部分切除术与穿孔修补术对急性胃穿孔患者围术期指标、并发症发生率的影响。方法选取2020年4月—2023年1月微山县人民医院收治的80例急性胃穿孔患者为研究对象,根据不同手术治疗方案分为胃大部分切除术组与穿孔修补术组,各40例。对比两组患者围术期相关指标以及并发症发生情况。结果胃大部分切除术组患者的治疗有效率(97.50%)高于穿孔修补术组(82.50%),差异有统计学意义(χ^(2)=5.000,P<0.05)。与胃大部分切除术组相比,穿孔修补术组的手术时间和住院时间更短,术中出血量更少,术后疼痛评分和并发症总发生率更低,差异有统计学意义(P均<0.05)。结论急性胃穿孔患者的治疗中,胃大部分切除术的疗效更优,但穿孔修补术可减少患者手术和住院时间,改善术中出血与术后疼痛情况,减少并发症的发生,在临床工作中应根据患者实际情况合理选择治疗方案。Objective To compare the effects of majority gastrectomy and perforation repair on perioperative indica⁃tors and complication rates in patients with acute gastric perforation.Methods 80 patients with acute gastric perfora⁃tion received by Weishan County People's Hospital from April 2020 to January 2023 were selected as the study ob⁃jects and were divided into majority gastrectomy group and perforation repair group according to different surgical treatment plan with 40 cases in each group.Perioperative indicators and complications were compared between the two groups.Results The effective rate in the majority gastrectomy group(97.50%)was higher than that in the perfora⁃tion repair group(82.50%),the difference was statistically significant(χ^(2)=5.000,P<0.05).Compared with the majority gastrectomy group,the perforation repair group had shorter time of surgery and hospital stay,less intraoperative bleed⁃ing,and lower postoperative pain score and total incidence rate of complications,and the differences were statistically significant(all P<0.05).Conclusion In the treatment of patients with acute gastric perforation,majority gastrectomy is better,but perforation repair can reduce the operation and hospital stay,and improve intraoperative bleeding and post⁃operative pain,reduce the occurrence of complications.In clinical work,the treatment plan should be reasonably se⁃lected according to the actual situation of patients.
