康复新液联合马应龙痔疮栓对肛瘘患者术后创面及生长因子的影响

摘　　要：目的:康复新液联合马应龙痔疮栓对肛瘘患者术后创面渗液评分、疼痛评分和血清血管内皮生长因子(VEGF)、碱性成纤维细胞生长因子(bFGF)水平影响。方法:选取肛瘘患者103例为研究对象,按照随机数字表法分为对照组51例、观察组52例,两组均采用常规治疗,对照组采用康复新液治疗,观察组采用康复新液联合马应龙痔疮栓治疗。对比治疗21 d后临床疗效,治疗后恢复情况,治疗前及治疗21 d后肛肠动力学检查,术后3、7、14、21 d创面渗液评分及疼痛评分,治疗前及治疗21 d后血清VEGF、bFGF水平。结果:治疗21 d后,观察组临床疗效为96.15%高于对照组84.31%(P<0.05),观察组愈合总时间低于对照组(P<0.05),Wexner评分低于对照组(P<0.05),两组肛肠动力学指标均较治疗前降低,且观察组上述功能均低于对照组(P<0.05)。两组患者血清VEGF、bFGF水平较治疗前显著升高,且观察组上述指标均高于对照组(P<0.05)。治疗21 d后两组渗液评分及VAS评分较治疗3 d后时显著降低,且观察组低于对照组(P<0.05),观察组术后3、7 d渗液评分及VAS评分低于对照组(P<0.05)。结论:康复新液联合马应龙痔疮栓治疗肛瘘,能有效减少患者创面渗液,并缓解患者疼痛,上调血清VEGF、bFGF水平,有助于促进创面快速愈合。Objective:To explore effect of Kangfuxin solution combined with Mayinglong hemorrhoids suppository on postoperative wound exudate score,pain score and serum VEGF and bFGF levels in patients with anal fistula.Methods:103 patients with anal fistula were selected as research subjects and divided into control group(51 cases)and observation group(52 cases)according to the random number table method.Both groups were given conventional treatment.The control group treated with Kangfuxin solution,and the observation group given Kangfuxin solution combined with Mayinglong hemorrhoids suppository.The clinical efficacy after 21 days of treatment,recovery status after treatment,anorectal dynamics examination before treatment and after 21 days of treatment,wound exudate score and pain score at 3,7,14 and 21 days after surgery levels of serum VEGF,bFGF before treatment and after 21 days of treatment were compared between both groups.Results:After 21 days of treatment,the clinical efficacy in the observation group 96.15%was significantly higher than that in the control group 84.31%(P<0.05).The total healing time in the observation group after treatment was significantly shorter than that in the control group(P<0.05),while the Wexner score were significantly lower than those in the control group(P<0.05).All of the anorectal dynamics indicators in both groups were significantly reduced compared with those before treatment,and the above indicator lower in the observation group(P<0.05).Serum VEGF and bFGF levels were significantly risen in both groups compared with those before treatment,and the levels in the observation group were higher than those in the control group(P<0.05).At 21 days of treatment,the exudate score and VAS score in the two groups were significantly declined compared with those at 3 days of treatment.The two scores were lower in the observation group than those in the control group(P<0.05),and the exudate score and VAS score in the observation group at 3 and 7 days after surgery were significantly lower than thos
