摘　　要：半身不遂是脑卒中(CVA)的常见临床证候,CVA中医称之为中风,历代医家对中风半身不遂多有论述,其证候名历经了偏枯、手足不遂、半身不遂的演变。半身不遂具有猝然起病和一侧肢体运动功能障碍特点,可见于中风、痹证、痿证,临证根据证候和疾病特点进行鉴别。CVA半身不遂可分为实证、虚证、虚实夹杂三类,实证证素以风、火、痰、瘀为主,虚证证素以气虚、血虚、阴虚为主,各证素可单独为病也可相兼为病,由此构成CVA半身不遂临床病机的复杂性。历代医家通过继承与创新对中风半身不遂的理论逐渐完善,其学术思想与辨证用药各有特点。Hemiplegia is a common clinical symptom of cerebrovascular accident(CVA).CVA is called apoplexy in Chinese medicine.Doctors of all ages have discussed hemiplegia in apoplexy,and its symptoms have evolved from Piankuparalysis of the limb and hemiplegia.Hemiplegia has the characteristics of sudden onset and motor dysfunction of one limb,which can be seen in apoplexy,arthralgia syndrome and flaccidity syndrome.Clinical symptoms can be distinguished according to the symptoms and characteristics of the disease.CVA hemiplegia can be divided into three categoriesexcess syndromedeficiency syndrome and syndrome of deficiency and excess.The excess syndrome mainly consists of wind,fire,phlegm and blood stasis,while the deficiency syndrome mainly consists of Qixu Xuexu and Yinxu.Each syndrome element can be single disease or combination of diseases,which constitutes complexity of clinical pathogenesis of cerebrovascular accident hemiplegia.Through inheritance and innovation,doctors of successive dynasties gradually perfected the theory of apoplexy hemiplegia.Their academic thoughts and dialectical drug use have their own characteristics,and combined with current clinical theory,these theories worthy of further development and exploration.
