推拿联合肌筋膜触发点针刺原发性痛经疗效研究

摘　　要：目的:观察推拿联合肌筋膜触发点针刺治疗原发性痛经(PD)的临床疗效。方法:选择80例PD患者,随机分为两组,每组40例。对照组予常规止痛治疗,观察组予以推拿联合肌筋膜触发点针刺治疗,对比两组疗效。结果:经治疗,观察组痛经症状评分显著低于对照组(P<0.05)。两组患者治疗后及随访3个月时视觉模拟评分(VAS)、现时疼痛强度评分(PPI)、痛经症状量表评分(CMSS)均较治疗前显著下降(P<0.05),但对照组随访3个月时VAS评分、PPI评分及CMSS评分较治疗后显著升高(P<0.05)。治疗后及随访3个月时观察组VAS评分、PPI评分及CMSS评分均显著低于对照组(P<0.05)。治疗后,观察组患者血清雌二醇(E2)水平显著低于对照组,前列腺素(PGE2)水平显著高于对照组(P<0.05)。治疗后,观察组收缩期血流速度(Vs)及舒张期血流速度(Vd)、平均速度(Vm)显著高于对照组(P<0.05)。结论:推拿联合肌筋膜触发点针刺治疗原发性痛经,能有效调节患者前列腺素及孕激素水平,改善其子宫微循环状态,减轻疼痛不适,提高临床疗效,且作用更加持久,远期效果更好。Objective:To observe clinical efficacy of massage combined with myofascial trigger point acupuncture in treatment of primary dysmenorrhea.Methods:80 patients with primary dysmenorrhea were randomly divided into two groups,with 40 cases in each group.The control group received routine pain relief treatment,while the observation group received massage combined with myofascial trigger point acupuncture for treatment.The therapeutic effects of the two groups were compared.Results:Compared with the the control group,the symptom score of dysmenorrhea in the observation group was significantly lower after treatment(P<0.05).After treatment and at 3 months of follow-up,the VAS score,PPI score,and CMSS score of both groups decreased significantly compared to before treatment(P<0.05).The VAS score,PPI score,and CMSS score in the control group increased significantly at 3 months of follow-up(P<0.05).Compared with the control group,the VAS score,PPI score,and CMSS score in the observation group were significantly lower after treatment and at 3 months of follow-up(P<0.05).Compared with the control group,the E2 level of patients in the observation group was significantly lower,while the PGE2 level higher after treatment(P<0.05).Compared with the control group,the levels of Vs,Vd,and Vm in the observation group were significantly higher after treatment(P<0.05).Conclusion:Massage combined with myofascial trigger point acupuncture can effectively regulate the levels of prostaglandins and progesterone in patients with primary dysmenorrhea,improve uterine microcirculation symptoms,alleviate pain discomfortand clinical efficacy.The effect is more long-lasting and the long-term effect is better.
