加味左金丸治疗肝胃郁热型功能性消化不良临床疗效及对相关激素水平的影响

摘　　要：目的观察加味左金丸治疗肝胃郁热型功能性消化不良的临床疗效及对相关激素水平的影响。方法研究纳入南京市中西医结合医院脾胃病科门诊治疗的120例肝胃郁热型功能性消化不良患者(2021年1月—2023年1月),将患者以随机数字表法分为对照组(60例)与治疗组(60例),给予对照组患者多潘立酮片治疗,给予治疗组患者多潘立酮片结合加味左金丸治疗,各组患者临床数据观察:临床治疗效果、治疗前后中医证候(上腹痛、餐后饱胀、上腹烧灼感、嗳气等)积分变化、上腹痛、餐后饱胀、上腹烧灼感、嗳气等证候改善时间、治疗前后患者胃泌素与胃动素水平变化、血清白介素-6(interleukin 6,IL-6)及肿瘤坏死因子α(tumor necrosis factor-α,TNF-α)水平变化、尼平消化不良指数(nephrology dialysis transplantation, NDT)及生活质量量表(short form 36 questionnaire, SF-36)评分变化、不良反应。结果治疗组患者治疗总有效率比对照组高[95.00%(57/60)vs 83.33%(50/60)](P<0.05);治疗组患者上腹痛、餐后饱胀、上腹烧灼感、嗳气等证候改善时间均短于对照组患者(P<0.05);治疗前各组患者中医证候(上腹痛、餐后饱胀、上腹烧灼感、嗳气等)积分、胃泌素与胃动素水平、IL-6及TNF-α水平、NDT及SF-36评分比较(P>0.05),治疗后各组患者中医证候(上腹痛、餐后饱胀、上腹烧灼感、嗳气等)积分、胃泌素与胃动素水平、IL-6及TNF-α水平、NDT及SF-36评分均改善,治疗组患者治疗后中医证候(上腹痛、餐后饱胀、上腹烧灼感、嗳气等)积分、胃泌素与胃动素水平、IL-6及TNF-α水平、NDT及SF-36评分均优于对照组(P<0.05);治疗组患者不良反应与对照组不良反应均较少(P>0.05)。结论加味左金丸治疗肝胃郁热型功能性消化不良的临床疗效显著,可较好改善患者胃肠功能,促进患者更快恢复健康,且患者生活质量提升,治疗不良反应少,安全可靠。Objective To observe the clinical effect of Jiawei Zuojin Pill(加味左金丸)on functional dyspepsia of heat stagnation in liver and stomach type and its influence on related hormone levels.Methods A total of 120 patients with functional dyspepsia due to heat stagnation in liver and stomach(from January 2021 to January 2023)in the Spleen and Stomach Diseases Department of Nanjing Integrated Traditional Chinese and Western Medicine Hospital were enrolled in this study.The patients were divided into control group(60 cases)and treatment group(60 cases)by random number table method.The patients in the control group were treated with Domperidone Tablets.Those in the treatment group were given Domperidone Tablets combined with Jiawei Zuojin Pill.The clinical treatment effect,integral changes of TCM syndrome(epigastric pain,postprandial fullness,epigastric burning sensation,belching,etc.)before and after treatment,the improvement time of epigastric pain,postprandial fullness,epigastric burning sensation,belching and other symptoms,the level changes of gastrin and motilin of patients before and after treatment,the levels of serum interleukin-6(IL-6)and tumor necrosis factorα(TNF-α),nephrology dyspepsia index(NDT),short form 36 questionnaire(SF-36),score changes and adverse reactions of two groups were observed.Results The total effective rate of the treatment group was higher than that in the control group[95.00%(57/60)vs 83.33%(50/60)](P<0.05).The improvement time of upper abdominal pain,postprandial fullness,burning sensation in the upper abdomen and belching in the treatment group was shorter than that in the control group(P<0.05).Before treatment,the scores of TCM symptoms(upper abdominal pain,postprandial fullness,burning sensation in the upper abdomen,belching,etc.),the levels of gastrin,motilin,IL-6 and TNF-α,NDT and SF-36 scores not significant in both groups(P>0.05).After treatment,the scores of TCM symptoms(upper abdominal pain,postprandial fullness,burning sensation in the upper abdomen,belching,etc.),the
