针刀治疗肛周疾病术后顽固性疼痛1例报告

摘　　要：术后疼痛是外科手术治疗肛肠疾病最常见的并发症,不利于患者预后。报道1例肛周疾病术后顽固性疼痛患者的诊治过程,分析其顽固性疼痛可能与患者原发病未能彻底根治、反复手术操作刺激、年龄、饮食及情绪等因素有关。结合患者病情,综合考虑其主要原因为多次术后损伤形成瘢痕,导致内括约肌痉挛引发疼痛。针刀疗法通过部分切断内括约肌解除痉挛,同时刺激穴位调经通络、行气活血以缓解患者疼痛,具有创面小、疗效显著及术后并发症少等优势。为中医特色疗法在肛肠疾病及术后并发症的应用提供新思路。Postoperative pain is the most common complication of anorectal diseases treated by surgery,and is poor prognosis.One patient with intractable pain after perianal surgery was reported about the treatment process to analyze that the intractable pain may be related to the failing to completely cure the primary disease,repeated surgical stimulation,age,diet and emotion.According to the patients’condition,the main reason might be that multiple postoperative injuries lead to scar forming,resulting in internal sphincter spasm and pain.Acupotomy therapy relieves spasm by partially cutting the internal sphincter,and at the same time stimulates acupoints to regulate meridians and dredge collaterals,promote qi and activate blood to relieve pain.It has the advantages of small wound,significant curative effect and less postoperative complications.It provides new ideas for the application of traditional Chinese medicine characteristic therapy in anorectal diseases and postoperative complications.
