摘 要:目的:探讨微骨折技术联合自体富血小板血浆(PRP)治疗HeppleⅤ期距骨骨软骨损伤的临床效果。方法:60例HeppleⅤ期距骨骨软骨损伤患者随机分组。对照组(30例)患者采用微骨折技术进行手术治疗,观察组(30例)患者采用微骨折技术联合富血小板血浆注射治疗,比较两组临床疗效。结果:术后12个月,两组患者踝关节疼痛视觉模拟量表(VAS)评分均较术前明显降低,且观察组低于对照组,差异有统计学意义(P<0.05);美国足与踝关节协会(AOFAS)踝与后足功能评分、踝关节活动度及简明健康状况调查表(SF-36)评分均较术前升高,且观察组高于对照组,差异有统计学意义(P<0.05);观察组并发症发生情况与对照组比较,差异无统计学意义(P>0.05)。结论:对于HeppleⅤ期距骨骨软骨损伤患者,应用微骨折技术联合富血小板血浆治疗具有显著疗效,能够减轻踝关节疼痛症状,促进踝关节功能恢复,提高生活质量,且安全性良好。Objective:To investigate the clinical efficacy of microfracture combined with autologous platelet-rich plasma(PRP)on osteochondral injury of talus of Hepple stage Ⅴ.Methods:60 patients with stage Ⅴ talus osteochondral injury of Hepple were randomized.The control group(30 cases)was treated with microfracture technology,and the observation group(30 cases)was treated with microfracture technology combined with PRP injection.The clinical efficacy of the two groups was compared.Results:At 12 months after surgery,the visual analogue scale(VAS)scores of ankle pain in both groups were significantly lower than that before surgery,and the observation group was lower than the control group(P<0.05).The American Foot and Ankle Association(AOFAS)ankle and hindfoot function scores,ankle range of motion and Short Form of Health Status Survey(SF-36)scores were higher than those before surgery,and the observation group were higher than the control group(P<0.05).There was no significant difference in the incidence of complications between observation group and control group(P>0.05).Conclusion:For Hepple stage Ⅴ patients with osteochondral injury of talus,the application of microfracture technology combined with PRP has a significant effect,which can effectively reduce the symptoms of ankle pain,promote the recovery of ankle function,improve the quality of life,and has good safety.
