负压联合银敷料治疗感染性创面的临床疗效

摘　　要：目的:探讨负压联合银敷料治疗感染创面的临床效果。方法:回顾性分析2022年1月至2023年1月感染性创面患者58例,随机分为联合治疗组和对照组各29例,对照组在机械/超声清创后采用传统无菌敷料治疗,联合治疗组在机械/超声清创基础上采用负压联合银离子敷料治疗。治疗第7天比较两组细菌培养阳性率、肉芽组织覆盖率和视觉模拟评分(VAS),记录两组创面愈合时间和治疗总有效率。结果:联合治疗组伤口细菌培养阳性率为10.34%(3/29),明显低于对照组[34.48%(10/29)](P<0.05);联合治疗组肉芽组织覆盖率为(86.97±3.78)%,明显高于对照组[(70.66±1.75)%](P<0.05);联合治疗组伤口VAS评分为(1.83±0.80)分,明显低于对照组[(3.76±0.95)分](P<0.05),联合治疗组创面愈合时间为(16.93±2.34) d,明显少于对照组[(22.41±2.20)d](P<0.05)。联合治疗组治疗总有效率为93.10%(27/29),明显高于对照组[72.41%(21/29)](P<0.05)。结论:负压联合银敷料治疗感染创面,可有效控制伤口感染、促进肉芽生长、减轻患者疼痛和缩短创面愈合时间。Objective:To investigate the clinical effect of negative pressure combined with silver dressing in the infected wounds treatment.Methods:A retrospective analysis was performed on 58 patients with infectious wounds from January 2022 to January 2023,and the patients were randomly divided into the combined treatment group and control group,with 29 cases in each group.The control group was treated with traditional sterile dressing after mechanical/ultrasonic debridement,and the combined treatment group was treated with negative pressure combined with silver ion dressing based on me⁃chanical/ultrasonic debridement.On the 7th day of treatment,the positive rate of bacterial culture,the coverage rate of granulation tissue and the visual analogue scale(VAS)scores of the two groups were compared,and the wound healing time and the total effective rate of treatment were recorded.Results:The positive rate of bacterial culture in the combined treatment group was 10.34%(3/29),which was significantly lower than the rate of 34.48%(10/29)in the control group(P<0.05).The granulation tissue coverage rate of the combined treatment group was(86.97±3.78)%,which was significantly higher than the rate of(70.66±1.75)%in the control group(P<0.05).The wound VAS score of the combined treatment group was 1.83±0.80 points,which was significantly lower than the score of 3.76±0.95 points in the control group(P<0.05),and the wound healing time of the combined treatment group was significantly less than in the control group as(16.93±2.34)d vs(22.41±2.20)d(P<0.05).The total effective rate of the combined treatment group was 93.10%(27/29),which was significantly higher than the rate of 72.41%(21/29)in the control group(P<0.05).Conclusion:Negative pressure combined with silver dressing can effectively control wound infection,promote granulation growth,reduce pain,and shorten wound healing time.
