摘　　要：目的 探讨分期法乳房悬吊联合假体置入矫治乳房下垂在临床治疗中的效果。方法 回顾性分析自2020年11月至2023年12月,大连医科大学附属第一医院整形外科收治24例(48侧)轻、中度乳房下垂的求美者,根据手术方式分为同期手术组(n=14)和分期手术组(n=10),同期手术组同时行隆乳术联合乳房悬吊术和双环法乳房上提术,分期手术组一期行隆乳术,二期行乳房悬吊术和双环法乳房上提术。比较两组求美者手术前后的乳房形态指标、并发症发生率,以及求美者满意度情况。结果 术后3个月,两组求美者手术前后胸乳间距、乳头至下皱壁最低点间距、乳头至正中线距离比较,差异均有统计学意义(P<0.05)。分期手术组术后并发症发生率低于同期手术组(P<0.05)。分期手术组满意度评分高于同期手术组(P<0.05)。结论 采用分期法乳房悬吊联合假体置入矫治乳房下垂,术后乳房松垂改善明显,乳房形态佳,且并发症发生率较低,求美者满意度较高,具有良好的临床治疗效果。Objective To investigate the effect of staged breast suspension combined with prosthesis implantation in the clinical treatment of breast ptosis.Methods A retrospective analysis of 24 candidates with mild or moderate breast ptosis who underwent surgical correction in the author's hospital from November 2020 to December 2023 was performed,and they were divided into the simultaneous surgery group(n=14)and the staged surgery group(n=10)according to the surgical plan at that time.The simultaneous surgery group underwent breast implantation combined with breast suspension and Bi-loop mastopexy at the same time,while the staged surgery group underwent breast implantation in one stage and breast suspension combined with Bi-loop mastopexy in the second stage.The preoperative and postoperative breast morphology indexes,complication rates,and patient satisfaction were compared between the two groups.Results In the staged surgery group and the concurrent surgery group,the differences in the preoperative and postoperative thoracic breast spacing,nipple to the lowest point of the inferior crural wall spacing,and nipple to median distance were statistically significant(P<0.05).The incidence of postoperative complications was lower in the staged surgery group than in the concurrent surgery group(P<0.05).Satisfaction scores in the staged surgery group were higher than those in the concurrent surgery group and were statistically different(P<0.05).Conclusion The use of staged method of the breast suspension combined with implantation of prosthesis to correct breast ptosis has obvious improvement of breast ptosis,good breast shape,and low complication rate and high satisfaction of candidates,which is a very good clinical therapeutic effect.
